Objective:To explore the difference of the white membrane formed after tonsillectomy with coblation and traditional tonsillectomy, and whether the white membrane can be used as an observation index of wound recovery. Methods:In the first part,60 patients with tonsillectomy were randomly divided into two groups: 30 patients with coblation tonsillectomy(observation group)and 30 patients with traditional tonsillectomy(control group). The formation of white membrane, time of white membrane formation and time of falling off were compared between the two groups. In the second part,30 mice were selected. The oral mucosa on both sides of the mice were prepared by coblation and traditional operation respectively. The pathological sections were made after obtaining the white membrane, and the histomorphological characteristics of the white membrane were compared.Use spss22.0 statistical software to analyze the data. The counting data is expressed by rate, and the comparison between groups is analyzed by Chi-square test; The data with normal distribution was described by sx ± and two independent sample t-tests were used for comparison between groups;medians(interquartile range)were used for data that did not meet normality, and rank sum tests were used for comparison between two groups. Results were statistically significant at P<0.05.Results:1.Clinical experiment: the formation of white membrane in the observation group is better than that in the control group(P<0.05);the formation time of white membrane in the observation group is earlier than that in the control group(P<0.05);the start and complete fall off time of white membrane in the observation group is later than that in the control group(P<0.05).2.Animal experiment: under the microscope of white membrane formed by coblation, there are no obvious exudation and necrosis on the surface,a large number of glassy collagen, and chronic inflammatory cell infiltration can be seen inside.Under the microscopeof white membrane formed by traditional operation, there were erosion, exudation and necrosis of mucous membrane, infiltration of neutrophils and some lymphoplasmacytes, hyperplasia of fibrous tissue and myxosis of stroma. Conclusion:Coblation tonsillectomy has the advantages of early formation of the white membrane and good growth of the white membrane, which is helpful for the recovery of patients after operation;however, the time of falling off of the white membrane is longer than that of traditional tonsillectomy.Through the analysis of pathological results of the white membrane,it is suggested that the white membrane has more advantages in the prevention of infection after tonsillectomy.
