Meta-analysis Of Coblation Versus Electrocoagulation For Tonsillectomy

Background Tonsillectomy is one of the most commonly performed procedures in otolaryngology. There are several operative methods currently in use, but the superiority of one over another has not been clearly demonstrated.Objective To assess the effectiveness and safety of coblation tonsillectomy compared with electrocoagulation tonsillectomy.Methods Pubmed, Embase, Cochrane Library, SinoMed, Wanfang Data, VIP and CNKI were searched by computer, supplemented relevant conference proceedings, equipment manufacturers and reference lists of al1 literature by manual retrieval. All the literatures about the two operation methods were compared. Coblation tonsillectomy and electrocoagulation tonsillectomy were considered as the treatment group and the control group, respectively. The quality of the methodology was evaluated by two independent observers. The efficient data were abstracted and statistically analyzed.Results 13 trials were identified, including 10 randomized controlled trials and 3 case-control studies. Meta analysis indicates that compared with electrocoagulation tonsillectomy, coblation tonsillectomy could reduce the VAS score[WMD=-1.15,95%CI(-1.60,-0.70),P<0.00001]. There was no statistically significant difference in the surgery time[WMD=0.14,95%CI(-1.08,1.36),P=0.82], the amount of intraoperative blood loss[WMD=-1.19,95%CI(-2.58,0.19),P=0.09], postoperative hemorrhage[OR=1.56,95%CI(0.67,3.65),P=0.30]and recovery period[WMD=0.70,95%CI(-0.07,1.47),P=0.07],[WMD=-1.58,95%CI(-3.95,0.79),P=0.19].Conclusion Current evidences suggested that coblation tonsillectomy in the pain score was superior to electrocoagulation tonsillectomy. However, more large, multi-center, high-quality studies are required to compare the outcomes of these two procedures.
