Objective: To compare guillotine tonsillectomy and coblation tonsillectomy wound recovery after tonsillectomy in child patents(intra-operative blood loss,post-operative blood loss, and duration of post-operative pain, occurrence rate and degree of postoperative oral malodor)to select the optimal surgical method. Methods: Selecting patients(from January 1,2016 to June 1,2018,97)with obstructive sleep apnea syndrome(OSAS)due to tonsillar hypertrophy,The age range was 2 to 8years.According to the way of operation, they were divided into observation group(49cases)and control group(48 cases).guillotine tonsillectomy were included into the observation group, while coblation tonsillectomy were used as controls group.Visual analogue scale(VAS)was used to record the pain scores and pain relief time on the 1,3,7,15 and 30 st after operation in the two groups, and the duration of oral odour after operation was recorded, and the intraoperative and postoperative bleeding volume was estimated in the two groups. The complete detachment time of tonsillar fossa pseudomembrane was recorded in two groups, and the tonsillar capsule thickness was measured by HE staining after operation in two groups.SPSS22.0 statistical software was used to analyze and compare the index difference between the two groups. Results: There was no significant difference in pain score between the observation group and the control group on the 1st day after operation(P > 0.05),but on the 3,7,15 and30 st after operation, the pain score and the oral odour score after the operation were statistically significant(P<0.05).There were significant difference in the time of complete detachment of pseudomembrane in tonsillar fossa and the thickness of tonsil capsule after operation(P<0.05).There were more bleeding in the observation group than in the control group(P<0.05),but the incidence of postoperative bleeding was lower in the observation group than in the control group. Conclusion: Compared with coblation tonsillectomy, guillotine tonsillectomy obviously relieve postoperative pharynx pain, postoperative oral odour, postoperative tonsillar fossa pseudomembranes fall off faster,There is more bleeding in tonsillectomy than that in hypothermic plasma resection, but it is still within the normal range, and it can reduce the incidence of postoperative bleeding. Therefore traditional tonsillectomy by guillotine still has its application value in clinical practice.
