Objective:Colorectal adenoma is an important precancerous lesion of colorectal cancer.Understanding the risk factors of colorectal adenoma is of great significance for the prevention and treatment of colorectal cancer.This study analyzed the relationship between the occurrence of colorectal polyps and serum lipid and uric acid in Fujian,retrospectively,and expected to understand the influencing factors of the occurrence of colorectal polyps,so as to provide guidance for reducing the occurrence of colorectal cancer.Methods:563 patients with colorectal polyps,intestinal dysfunction and irritable bowel syndrome diagnosed by electronic colonoscopy during hospitalization in the First Affiliated Hospital of Fujian Medical University,from January 2016 to October 2018,were enrolled in this study.The clinical data of 563 patients were recorded and analyzed retrospectively.The subjects were divided into the colorectal polyps group and the control group according to electronic colonoscopy.The baseline data between the two groups were compared by T test,Rank sum test and Chi-square test.Then,all subjects were further stractified by gender,The difference between the two groups in diferent gender were also contrasted;Finally,All subjects with colorectal polyps were stratified according to the size,number,morphology and pathology of colorectal polyps,the serum lipid and uric acid level were compared between each two groups,binary logistic analysis were performed to explored the relationship between serum lipid and uric acid and the development of colorectal polyps.Results:1.563 patients enrolled,were divided into colorectal polyps group(including 412 colorectal polyps patients)and control group(151 no-colorectal polyps subjects),Compared with control group,The proportion of males(74.8% vs 39.1%,P < 0.05),the proportion of family history of polyps was higher(6.8% vs 2%,P < 0.05),and the levels of triglyceride(TG),very low densitylipoprotein cholesterol(VLDL-C),apolipoprotein B(Apo B)and uric acid(UA)were also higher(P< 0.001).However,the level of high density lipoprotein cholesterol(HDL-C)was lower than that in the control group(P<0.05),the difference was statistically significant.Binary Logistic Regression analysis showed that hypertriglyceridemia(P = 0.025,OR = 2.555,95% CI: 1.122-5.816),gender(P = 0.000,OR = 4.746,95% CI: 2.920-7.713),Apo B level(P = 0.026,OR = 2.742,95% CI: 1.128-6.667),family history(P = 0.030,OR = 3.986,95% CI: 1.146-13.861)were risk factors for colorectal polyps.2.All 563 patients enrolled were stratified according to gender,Among 367 male subjects,308 were enrolled into colorectal polyps group and 59 enrolled into control group.Compared with the control group,the levels of TG(P<0.05)and VLDL-C(P<0.05)were higher in the male polyps group.There was no significant correlation between colorectal polyp and uric acid(P>0.05).The logistic analysis showed hypertriglyceridemia(P=0.039,OR=3.532,95% CI:1.063-11.736)was a risk factor for the development of colorectal polyps in males.Among 196 female subjects,colorectal polyps group included 104 patients,and control group enrolled other 92 subjects.Apolipoprotein A1(Apo A1)level in the female polyp group was higher than that in control group(P<0.05).There was no significant correlation between colorectal polyp and serum lipid and uric acid(P>0.05)in female.3.412 cases of colorectal polyps were enrolled.Stratification analysis according to the number of polyps showed that,the UA level in the multiple polyps group was higher than that in the single polyps group(P<0.05).However,there was no significant correlation between hyperuricemia(P>0.05)and polyps.Stratification analysis according to the morphology of polyp discovered that LDL-C and Apo-B levels in the protuberant polyp group were higher than those in the flat polyp group(P <0.05).4.364 patients with polyps were divided into adenomatous polyps group(215 cases of high-risk adenomas and 76 cases of low-risk adenomas)and non-adenomatous polyps group according to pathological types.There was no significant difference in serum lipid level and uric acid level between the two groups(P >0.05).Difference between high-risk and low-risk adenomas group also have not been discovered in serum lipid level and uric acid level(P > 0.05).5.Compared with non-adenomatous polyps group,the positive rate of stool occult blood test in adenomatous polyps group was higher(90.3% vs 76.2,P < 0.05).Conclusion:1.Colorectal polyps are more likely to occur in males and patients with a family history of polyps.2.High triglyceride level,high uric acid level and low high density lipoprotein cholesterol level are more likely to cause colorectal polyps.Triglyceridemia is a risk factor for male colorectal polyps.There was no significant correlation between serum lipid,uric acid levels and the occurrence of colorectal polyps in female.3.The uric acid level of multiple polyps was higher than that of single polyps,and the LDL-C and Apo-B levels of protuberant polyps were higher than that in the flat polyps.The level of serum lipid had little effect on the size,number,pathological type and risk stratification of colorectal polyps.4.The positive rate of stool occult blood test was higher in colorectal adenomas.
