Objective: This study explored the relationship between the TCM syndrome type of Chronic atrophic gastritis(CAG)and Intestinal metaplasia(IM)and pepsinogen(PG),making the TCM syndrome of this disease objective. The research is more perfect,in order to help realize the early diagnosis and early treatment of this disease, and to give full play to the advantages of integrated diagnosis and treatment of this disease. Methods: Collect 300 patients with CAG and IM in our hospital, take Helicobacter pylori(Hp)test, measure serum PG content, fill out a unified questionnaire, collect four diagnostic data for TCM dialectical classification, organize data, and draw conclusions.Results:1.General data:(1)The proportion of males and females in 300 CAG patients with IM was 1.16:1.(2)The middle-aged are the most. There was no statistically difference in the incidence of CAG with IM between different ages and sexes(P>0.05).(3)Spleen-stomach-damp-heat syndrome type is the most commonly seen, followed by spleen-stomach deficiency syndrome, and at least stomach-yin deficiency syndrom and qi-stagnation syndrome of liver and stomach.(4)The proportion of mediate intestinal metaplasia is the highest.(5)The positive rate of Hp was 54.33%.2.Factors and TCM syndromes:(1)There was no significant difference in the syndrome type and sex of CAG with IM.(2)There was significant difference in the syndrome type and age of CAG with IM(P<0.05).Spleen-stomach-damp-heat syndrome is common in young and middle-aged people. In the elderly, the syndrome of spleen-stomach deficiency is common.(3)There is a significant difference between different syndromes and intestinal metaplasia degree(P<0.05).Light and moderate intestinal metaplasia have the highest proportion of spleen-stomach-damp-heat syndrome, while severe intestinal metaplasia is more common with spleen-stomach deficiency syndrome, followed by spleen-stomach-damp-heat syndrome and stomach stasis blood syndrome.(4)There were statistically significant differences in Hp infection rates among different syndromes(P<0.05).Most of the syndromes were spleen-stomach-damp-heat syndrome, while stomach-yin deficiency syndrome was the least. There was no statistically difference in intestinal metaplasia degree and Hp infection rate(P>0.05).3.TCM syndromes?intestinal metaplasia and pepsinogen:Stomach stasis blood syndrome in PG?and PGR content is low,stomach-yin deficiency syndrome,spleen-stomach deficiency syndrome such as PG??PG? content is low compared with other syndromes.PG? ? PG? ? PGR content: light intestinal change>moderate intestinal>severe intestinal,in the process of pathological evolution, showed a trend of gradual decline.Conclusions:1.In Fuzhou area,spleen-stomach-damp-heat syndrome ? moderate intestinal metaplasia is common. Most of the young and middle-aged people are Spleen-stomach-damp-heat syndrome.Elderly people are often seen with spleen-stomach deficiency syndrome.Light intestinal metaplasia and moderate intestinal metaplasia have the highest proportion of spleen-stomach-damp-heat syndrome, while severe intestinal metaplasia is more common with spleen-stomach deficiency syndrome. The different syndrome types of TCM have its certain pathological basis.2.Stomach stasis blood syndrome in PG?and PGR content is low,stomach-yin deficiency syndrome,spleen-stomach deficiency syndrome such as PG?? PG? content is low compared with other syndromes. In the process of pathological evolution, showed a trend of gradual decline. The content of pepsinogen can be used as a reference index for judge the syndrome of TCM,help to guiding the clinical diagnosis and treatment in clinical works of significant difference.
