| Objective To investigate the relationship between plasma homocysteine level and carotid-femoral pulse wave velocity(cf PWV)in essential hypertensive patients.Methods A total of 515 patients(321 men and 194 women)were enrolled.Age,medical history,smoking history,history of alcohol intake as well as family history were asked.Body height and weight,systolic and diastolic blood pressure were recorded.Fasting food glucose,blood lipid profiles,Hb A1 c as well as homocysteine were determined.cf PWV was measured by Complior Analyse.Based on the cut-point of plasma homocysteine level(Hcy)and hypertensive condition,all subjects were divided into four groups: EH+HHcy group(n=196),EH group(n=133),HHcy group(n=81)and control group(n=105).Continuous variables were expressed as mean ± standard deviation(SD).Comparison among four groups were made by analysis of ANOVA and the comparisons between two groups were analysis of SNK.Pearson’s correlation analysis,partial correlation analysis and stepwise multiple regression were used to assess the association between cf PWV and homocysteine.Results 1、 In all subjects: The cfPWV in EH+HHcy group was significantly higher than other three groups.[(9.52±1.72)vs(8.85±1.33)、(8.66±1.42)、(8.01±1.25)m/s(P<0.05)].The cf PWV in HHcy group and EH group were significantly higher than control group(P<0.05=.However,there was no significant difference between EH group and HHcy group.The results also remained in young and middle-aged subjects(age range from 18 to 59).In aged subjects(age over 60 years old),the cf PWV in EH+HHcy group,EH group and HHcy group were higher than control group((10.02±1.89)、(9.43±1.41)、(9.19±1.54)vs(8.21±1.19)m/s),(P<0.05 =.There was no significant difference among other groups.2、The homocysteine had a positive relationship with cf PWV in subjects in follows groups: all subjects(0.286),hypertensive patients(0.248),female(0.371),young and middle-aged subjects(0.346),young and middle-aged hypertensive patients(0.361),young and middle-aged male(0.335)and aged women(0.327).3、Stepwise multiple regression analysis revealed that cf PWV was associated with systolic blood pressure,homocysteine and age in all subjects.In young and middle-aged hypertensive patients,men and aged women,higher homocysteine level was still the risk factor of increased cf PWV.However,homocysteine did not have any relationship with cf PWV in young and middle-aged nonhypertensive subjects and women and other aged subjects.Conclusion Age,blood pressure and plasma homocysteine level were determined factors of cf PWV.However,plasma homocysteine level was not related to the cf PWV in young and middle-aged as well as aged people with normal blood pressure.cf PWV of patiens with EH and HHcy was further higher than that of patients simply with EH or HHcy. |
