Objective:1.To investigate the prevalence and epidemiological characteristics of thyroid nodules(TN)in patients with abnomal glucose metabolism in Fujian coastal area.2.To explore the related risk factors that affect the prevalence of thyroid nodule in patients with abnormal glucose metabolism in Fujian coastal area.Methods:An epidemiological survey was carried out in Fujian coastal area from March to June in 2016 among 2530 resident living in cities or countryside.After signing the informed consent,the participants were asked to complete aquestionnaire,followed by screening tests including physical examination,a hematology test and thyroid ultrasonic examination.590 patients with abnormal glucose metabolism diagnosed based on inclusion and exclusion criteria were enrolled in the study.Participants were divided into the nodule group and non-nodule group according to thyroid ultrasonic test results.The parameters such as sex,age,body mass index(BMI),fast plasma glucose(FPG),haemoglobinA1C(HbA1c)and so on were compared in the two groups.Results:1.There were 250 cases of thyroid nodules in patients with abnormal glucose metabolism,and the overall prevalence of thyroid nodules was 42.4%.2.There were statistically significant differences in the distribution of gender,age,BMI,FPG,HbA1 cand hypertension between the thyroid nodule group and nonnodule group(P<0.05).3.The prevalence of thyroid noduleswith abnormal glucose metabolism in urban areas was higher than that in the rural areasP<0.05).4.The prevalence of thyroid nodules in females(47.3%)was significantly higher than that in males(37.9%)(P<0.05).5.In both male and female patients,the prevalence of thyroid nodules increased significantly with age(P<0.05).6.Patients with higher BMI(obesity group)appeared to have significantly higher prevalence of thyroid nodules compared to those with lower BMI(underweight group)(P<0.05).7.The prevalence of thyroid nodules in participants with FPGless than 6.0mmol/L,within 6.1-6.9mmol/L and more than 7.0mmol/L were 38.8%,42.0%,49.3%.The prevalence of thyroid nodules significantly increased with elevated FPG(P<0.05).8.The prevalence of thyroid nodules in participants with HbA1 c less than 6.5mmol/L(31.3%)was significantly higher than that in participants with HbA1 c more than 6.5mmol/L(46.5%)(P<0.05).9.Multifactor logistic regression analysis was performed by selecting the presence or absence of thyroid nodules as the dependent variable Y,and gender,age,hypertension,BMI,FPG and HbA1 c as the independent variables X.The results showed that the prevalence of thyroid nodules were positivecorrelated with the female sex,age,BMI and HbA1c(P<0.05),while no significant correlation was found between the prevalence of TN and hypertension or FPG(P>0.05).It’s concluded that the female sex,age,BMI and HbA1 cwere the independent risk factors of thyroid nodules in patientswith abnormal glucose.Conclusions:The female sex,age,BMI and HbA1 c were found to be the independent risk factors of thyroid nodules in patients with abnomal glucose metabolism in Fujian area.Therefore,for elderly female patients with abnormal glucose metabolism together with obesity and hyperglycemia,it’s important to take a routine thyroid ultrasonolography screening for the sake of early diagnosis and treatment of throid nodules.
