The Establishment,Cognition,Utilization And Evaluation Of Health Records In Main Urban Areas Of Chongqing:a Cross-sectional Study

Objective: Under the background of healthy China strategy,this paper aims to understand the status of health records of residents in the main urban area of Chongqing,by means of questionnaire survey on the awareness,establishment and use of health records,and analyze the factors affecting the establishment,awareness and use of health records,so as to provide empirical reference for the full coverage and full utilization of health records.Methods: The stratified proportional sampling method was used to conduct a questionnaire survey among 900 residents who had lived for more than 6 months in 36 streets of 9 districts in the main city of Chongqing.The statistical description is mainly relative number.The main methods of statistical inference are chi square test,disordered multi classification logistic regression model and binary logistic regression model.The main analysis is the relationship and significance between the personal and family factors of residents and the establishment,cognitionand utilization of health records.Results: a total of 848 valid questionnaires were collected.Among them,521 people have established health records,accounting for 61.5%,170 people have not established records,157 people are not clear,accounting for 20% and 18.5% respectively.The single factor analysis showed that the differences in the establishment of health files of different gender,age,education,occupation,household registration,housing type,and family economic self-assessment were statistically significant(P<0.05).Multiple logistic regression analysis results show that the gender,age,education,occupation,household registration,and housing type of residents are the influencing factors for the establishment of health records(P <0.05).The results of health records awareness analysis show that 637 people know health records,accounting for 75.1%,and 211 people do not know,accounting for 24.9%.Comparing the personal and family factors that may affect the residents' knowledge of health records,the results of single factor analysis showed that different gender,age,education background,occupation,household registration,housing type,family economic self-assessment had significant difference on the awareness rate of health records(P < 0.05).The results of binomial logistic regression analysis showed that the awareness rate of residents' health records was related to the gender,age,education,occupation,and household registration of the respondents(P <0.05);77.8% of the residents thoughtthat it was necessary to set up archives,53.7% of the residents paid attention to the content of health records,mainly focusing on the health examination information(52.4%).74.8% of the residents agreed with the sharing of electronic health records.The analysis of health records utilization showed that 337 of 521 residents had used health records,accounting for 64.7%,and 184 had not used health records,accounting for35.3%.The results of single factor analysis showed that there were significant differences in the utilization of health records among different genders,ages and occupations(P < 0.05);The results of binomial logistic regression analysis showed that gender was the influencing factor of the utilization of health records(P < 0.05);The utilization rate of periodic health check and chronic disease management is relatively high,accounting for 32.4% and 19.4% respectively.There were 15 people who did not intervene the risk factors,accounting for 4.5%.The main reason for not intervening was that they were busy with their work and had no time(76.2%);The health records of 46 respondents were not updated in time,accounting for 13.6%;In the survey of residents' evaluation of health file service,45 people were dissatisfied,accounting for 5.3%,9 people were very dissatisfied,accounting for 1.1%.the main reason is not satisfied with the publicity of health records(21.5%),health records in the personal information leakage(20.1%),and health records are not updated(20.8%).Conclusion: The construction rate,cognition rate and utilization rateof health records of residents in the main urban area of Chongqing need to be further improved.Gender,education and household registration are the influencing factors for residents to establish health records;The awareness of health records was affected by gender,age,education,occupation,household registration and other factors;The utilization of residents' health records was affected by gender;In addition,there are still some problems,such as insufficient awareness of the value of health records,insufficient publicity of health records,inadequate investment in personnel and technology to meet the needs of residents,weak security of health record information,and imperfect information sharing mechanism.Therefore,we should pay attention to the construction of male health records,narrow the differences between urban and rural health records,expand the popularity of health records among the low-educated people,pay attention to the people without fixed units,combine various modes of dissemination,deepen the propaganda of health records-related knowledge for residents,give full play to the leading position of the government,and increase the investment of talents,technology and capital policies.Strengthen the information construction of health records to ensure the orderly development of Chongqing residents' health records.
