Objective:To discuss the occurrence and development,clinical manifestations and prognosis of AIDS in children,so as to improve the attention of pediatrician's to AIDS on them in clinical work.Methods:90 patients who were suspected or diagnosed AIDS infection in the affiliated Children's Hospital of Chongqing Medical University from 2014.3 to 2019.8 were collected for clinical retrospective analysis.Results:Among the 90 cases,27/90 were diagnosed and 63/90were suspected.Among them,there were 48/90 males and 42/90females,with a proportion of about 1:1;the age varied from 1 day to 13 years and 3 months,the median age was 3 years and 11 months(3 years and 1 month~4 years and 10 months),the most cases were less than 6 years old.26/90 were from urban areas and 64/90 were from rural areas.Among the 90 patients,there were 13/90 patients from infection department,16/90 from pneumology department,17from neonatology department,7 from hematology department.There were 43/90 cases with a clear history of exposure during pregnancy,2/90 cases with a clear history of male sex.42/87 cases were explicitly breastfed or mixed-fed,and there was statistically difference between confirmed and suspected cases(?~2=6.521,P=0.011).The main clinical manifestations were fever,chronic diarrhea,repeated cough,pneumonia,hepatosplenic lymph node enlargement,anemia and so on.Among the related complications,the main manifestations of respiratory system were pneumonia,recurrent respiratory tract infection and pulmonary tuberculosis;the main manifestations of digestive system were thrush,recurrent oral ulcer,etc.;the main manifestations of blood system were anemia,thrombocytopenia and so on.In the laboratory examination,t here was statistically significant in liver function damage between the two groups(?~2=4.644,P=0.031),but there was no significant difference in myocardial markers,coagulation function and electrolyte disturbance between the two groups.A total of 31/90cases had complete lymphocyte classification counts,of which13/31 were from diagnosed group and 18/31 were from suspected group.According to the immune status classification,the immunosuppression of the 13/13 confirmed cases were middle to sever.11/27 diagnosed cases had clearly entered the stage of AIDS(40.7%).Conclusion:(1)The epidemic trend of AIDS in rural areas is obvious,mother-to-child transmission is still the main source of transmission of AIDS among children,but the transmission of adolescent male-to-male contact is gradually increasing,and more attention should be paid to this group of people in clinical work.(2)The clinical manifestations of AIDS in children were not specific.The main manifestation find expression in respiratory system,blood system and digestive system.(3)The rate of AIDS among children who is diagnosed in China is low,and the rate of those children who knows their infectious status is low,especially for infants and young children,The clinical control result is not ideal.(4)The rate of children with AIDS under virus suppression is low,the state of immunosuppression is poor,and the time of developing AIDS is significantly shorter than that of adults.
