Objectivevaricocele is one of the common diseases of male genitourinary system and the leading cause of male infertility.Through retrospective analysis,this study compares the clinical therapeutic effects of microscopic and laparoscopic varicocele ligation on patients with varicocele.MethodsA total of 120 male patients with varicocele diagnosed and treated by operation in the department of urology in our hospital from January 2016 to June 2019 were retrospectively analyzed.They were divided into laparoscopy group(n = 34)and microscope group(n = 86).Collect the clinical information of the patients,including(1)general data;(2)auxiliary examination indicators: results of color ultrasound of reproductive system and semen examination before and 3-4 months after operation;(3)operation related indicators: operation time,blood loss,postoperative complications;(4)symptom improvement,recurrence rate,average hospitalization days,total hospitalization expenses.And carry on the related statistical analysis.ResultsThere was no significant difference in age,varicose site,degree of varicose,maximum diameter of vein,VAS score of scrotal discomfort before operation and semen examination between microscope group and laparoscopy group(P>0.05).Compared with the laparoscopy group,the microscope group had longer operation time,shorter postoperative hospital stay and lower cost,and the difference was statistically significant(P <0.05).Compared with the semen examination before and after operation,there was no significant difference in semen volume and sperm density between the microscope group and before operation(P>0.05).However,there were significant differences in PH value,percentage of forward motile sperm,percentage of immobile sperm and percentage of normal morphological sperm(P < 0.05).The semen quality index in the laparoscopy group was also improved as compared with that before operation,but only the percentage of normal sperm was significantly increased(P<0.05).In comparison between groups,the improvement of sperm density,percentage of forward motile sperm and percentage of immobile sperm in microscope group was slightly better than that in laparoscopic surgery,but there was no significant difference between the two groups(P>0.05).There was no significant difference in the incidence of postoperative complications,recurrence rate and VAS score of postoperative scrotal discomfort between the two methods(P>0.05).Conclusionboth microscope and laparoscopic varicocele ligation can relieve the symptoms of scrotum pain,distension and discomfort in patients with varicocele,and help to improve the quality of semen.Compared with laparoscopic surgery,microscope surgery has shorter hospital stay and lower cost,and the incidence of postoperative complications and recurrence rate are also slightly lower.It has certain advantages in improving the decline of semen quality caused by varicocele.Therefore,under the condition of corresponding equipment and technology,it can be regarded as the priority mode of operation.
