Objective:To explore the correlation among the level of the serum Folate(FA),Vitamin B12(Vit B12),Homocysteine(Hcy),blood lipids and colon adenomatous polyps(AP)or early colorectal cancer,and to explore the diagnostic value of these indicators for AP and early colorectal cancer,so as to provide directions for the prevention and screening of AP and early colorectal cancer.Methods:38 patients with AP by colonoscopy and 20 patients with early colorectal cancer by colonoscopy and postoperative pathology were screened according to the inclusion criteria in the affiliated hospital of Qinhai university.At the same time,40 persons with normal intestinal mucosa who met the inclusion criteria were examined by electronic colonoscopy in the hospital.Basic information like age,gender,body mass index(BMI),family history of colorectal cancer(CRC),etc were recored.Related tests like determination of serum FA,Vit B12 and Hcy;Blood lipids measurements: total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein(HDL-C),apolipoprotein A(apo A),apolipoprotein B(apo B).Blood lipids derivatives(AIP,AC,CRI-1,CRI-2),faecal occult blood,colonoscopy histopathology(the test group should be improved),postoperative histopathology and enhanced CT of the entire abdomen and pelvis(the early colorectal cancer group should be improved)were completed.Statistical analysis: the relationship between AP and early colorectal cancer and serum FA,Vit B12,Hcy,blood lipids and its predictive value for AP and early colorectal cancer.Results:1.There was no significant difference in Hcy,FA and Vit B12 among the three groups(P > 0.05).2.An ordered logistic regression model was constructed for fecal occultic blood,CRC family history,TCH,TG,LDL-C,apo A,apo B,AC,AIP,CRI-1 and CRI-2.The AUC value of the model for AP was 0.674(P < 0.05).The AUC value of the model for early colorectal cancer was 0.882(P < 0.05).The AUC value of the model was 0.755(P < 0.05)for AP and early colorectal cancer differentiation.4.Diagnostic value of parallel connection between fecal occult blood and family history of CRC: the AUC value of parallel connection for AP was 0.622(P > 0.05);The AUC value of colonic carcinoma in parallel was 0.688(P < 0.05).The AUC value was 0.566 for AP and early colorectal cancer(P > 0.05).Conclusions:1.No association among the the level of the serum FA,Vit B12,Hcy,and AP or early colorectal cancer has been found.2.The model has diagnostic value for AP,and 50% of AP may be missed;The model has diagnostic value for early colorectal cancer.Blood lipids may be a factor to be considered in screening for AP and early colorectal cancer.3.The parallel connection of faecal occult blood and CRC family history is of diagnostic value for early colorectal cancer,and 50% of early colorectal cancer may be missed.
