Objective: To compare the changes of blood coagulation indexes of patients before and after surgery with VATS and traditional thoracotomy lung lobectomy,and to clarify the degree of influence of two different surgical methods on blood coagulation function of patients.Significance of this surgical method in radical operation of lung cancer in plateau area.Methods: The lung cancer patients admitted to the Department of Thoracic Surgery of Qinghai Provincial People's Hospital from September 2017 to October 2019 were selected as the research subjects.At the end of the experiment,a total of 65 patients were collected after screening according to the inclusion and exclusion criteria and divided into thoracoscopy 34 patients and 31 patients in the traditional thoracotomy group were collected 24 hours before surgery and early morning fasting venous blood on the first day,the third day,and the seventh day,and the changes of various coagulation indexes were measured: Determination of prothrombin time(PT)and activated partial thrombin time(APTT)using coagulation method;Determination of fibrinogen(FIB)and D-dimer(D-D)by latex immunoturbidimetry;Platelet(PLT)was measured by fluorescence method.SPSS20.0 software was used for statistical analysis and Graphpad Prism 7 was used for plotting.The normal distribution quantitative data were described by mean ± standard deviation,and the method was tested by t test.Qualitative data were described by relative Numbers,and 2 test was used for comparison between groups.Repeated measure variance analysis was used for comparison between groups before and after surgery.P < 0.05 was considered statistically significant.Results: All operations were successfully completed,and the patients were cured and discharged without death.Comparing and analyzing the general information(sex,age,weight)and general information(operation time,intraoperative blood loss)of the two groups of patients,the differences were not statistically significant(P> 0.05).Repetitive measurement variance analysis of relevant coagulation indicators before and after surgery was performed between the two groups of patients.It was found that the differences in PT and PLT at each time point between the two groups were statistically significant(P <0.001),and there was no statistically significant difference between the groups(P> 0.05),and there was no interaction between the time * groups(P> 0.05).The difference in APTT at each time point was statistically significant(P <0.001),the difference between the groups was not statistically significant(P> 0.05),and there was an interaction between time * groups(P <0.05).The difference between D-D at each time point was statistically significant(P <0.001),the difference between groups was statistically significant(P <0.05),and there was no interaction between time * groups(P> 0.05).The difference in FIB at each time point was statistically significant(P <0.001),the difference between groups was statistically significant(P <0.05),and there was an interaction between time * groups(P <0.05).Thirty-one patients in the traditional thoracotomy group had 1 patient with pulmonary thromboembolism(PTE)after surgery,and no venous thromboembolism(VTE)in the VATS group.Conclusion: 1.The blood after VATS and traditional thoracotomy was in a state of high coagulation.2.Compared with traditional thoracotomy,VATS has a relatively small effect on patients' coagulation function,and venous thromboembolism is less likely to occur after surgery.
