BackgroundThe thoracoscopy is one of the major breakthroughs in the department of thoracic surgery in twentieth century and is the representative of the minimally invasive surgery in the department of thoracic surgery.Thoracoscopic surgery using thoracoscopic surgery equipment and TV camera technology,small incision in the chest or casing to complete the complex minimally invasive department of thoracic surgery operation it has changed the most,the department of thoracic surgery disease treatment concept,is considered to be the end of twentieth century the department of thoracic surgery operation progress,is also the focus of the current development of department of thoracic surgery,With the improvement of the standard of living,the development and progress of medical technology.Videoassisted thoracoscopic surgery has become the first choice for the treatment of lung cancer.ObjectiveObjective to compare and analyze the clinical data of complete video-assisted thoracoscopic surgery(CVATS),video-assisted mini-thoracotomy(VAMT)and traditional thoracotomy in the treatment of lung cancer.MethodsSelected 83 cases of peripheral non-small cell lung cancer patients from March 2014 to June 2017 in Department of thoracic surgery,First Affiliated Hospital of Xinxiang Medical College,as the research object,the patient's general condition and were divided into three groups,complete video-assisted thoracoscopic surgery(CVATS)group,28 cases of video-assisted mini-thoracotomy(VAMT)group of 27 cases,traditional thoracotomygroup 28 cases.The operative time(min),intraoperative blood loss(ml),drainage tube indwelling time(d),postoperative flow rate(ml),lymph node number,postoperative hospital stay(d)and postoperative complications in three groups were compared and analyzed.ResultsThere were significant differences in the operation time,the amount of bleeding,the time of drainage tube placement and the time of hospitalization in the three groups(P <0.05).In group CVATS,intraoperative bleeding volume,drainage tube indwelling time,postoperative flow rate,postoperative hospitalization time was significantly less than that of VAMT group and thoracotomy group(P<0.05),VAMT group,intraoperative bleeding volume,drainage tube indwelling time,postoperative flow rate,postoperative hospitalization time was significantly less than the thoracotomy group(P<0.05);there was no significant difference in operative time and lymph node dissection between CVATS group,VAMT group and thoracotomy group(P>0.05).The incidence of complications in the CVATS group was significantly less than that in the thoracotomy traditional thoracotomy group(P<0.05).The incidence of complications in group CVATS was significantly lower than that in traditional thoracotomy group(P<0.05).There was no significant difference in the incidence of complications between the CVATS group and the VAMT group(P>0.05),and there was no significant difference between the group VAMT and the traditional thoracotomy group(P>0.05).ConclusionCompared with VAMT and traditional thoracotomy,CVATS has the advantages of small trauma,less complications and quick recovery.It can be used as the first choice for NSCLC patients in stage I-III A.
