Objective To investigate the relationship between preoperative peripheral blood fibrinogen levels(FIB),albumin to globin ratio(AGR),prognostic nutritional index(PNI)and clear cell renal cell carcinoma(ccRCC)prognosis.Methods This study collected 217 patients with clear renal cell carcinoma diagnosed in the urology department of the XXX Hospital from January 2011 to December 2014,and retrospectively analyzed the clinical data of ccRCC patients after partial nephrectomy or radical resection.Receiver Operating Characteristic Curve(ROC)was used to determine the optimal cutoff values of FIB,AGR and PNI in peripheral blood before surgery.Chi-square test was used to analyze the clinical data between groups differences in characteristics calculated using Kaplan-Meier method for survival rate.The log-rank test was used to analyze the gaps between components,and cox risk regression model used to analyze risk factors for each indicator.Results The postoperative follow-up time was 1-60 months,and the median follow-up time was 48 months.The optimal cutoff value was selected by ROC curveanalysis.The univariate analysis showed that FIB,AGR,PNI,tumor grade and Pathological staging were significantly correlated with patients' 5-year overall survival and disease-free survival(both P<0.01).Further analysis by Cox regression model,We found that FIB and AGR were not the5-year overall survivaland disease-free survival of ccRCC patients.About time-independent prognostic factor,PNI is an independent prognostic factor for the 5-year overall survival time of patients with ccRCC,but it cannot be used as an independent prognostic factor for disease-free survival time.Conclusion Preoperative peripheral blood PNI is an independent risk factor for poor prognosis in patients with ccRCC,while FIB and AGR cannot be used as indicators of independent prognosis in patients with ccRCC.
