Objective To investigate the risk factors of lumbar spinal epidural lipomatosis in obese patients.Methods A retrospective analysis of the data of 2048 patients who underwent MRI examination for the symptoms of low back pain from January 2015 to January 2018 in the General Hospital of Ningxia Medical University.According to the inclusion and exclusion criteria,129 cases were finally selected.According to the presence or absence of fat deposits in the epidural space of the Lumbar spinal canal,the selected patients were divided into two groups: the lumbar spinal epidural lipomatosis(LEL)group and the non-lumbar epidural lipomatosis group.The differences between the two groups were analyzed for the following variables: age,sex,body mass index(BMI),blood pressure(systolic blood pressure SBP,diastolic blood pressure DBP),fasting blood glucose(GLU),blood lipids(triglycerides(TG),total cholesterol(TC)),lipoproteins(high density lipoprotein(HDL),low density lipoprotein(LDL),apolipoprotein A1(APOA1),apolipoprotein B(APOB)),atherosclerosis index,serum uric acid(UA),ABO blood group,smoking,drinking,and previous diseases(essential hypertension,diabetes,cerebrovascular disease,peripheral atherosclerotic disease,gallbladder disease,fatty liver,obstructive apnea syndrome,thyroid disease,osteoarthritis),the data were analyzed by SPSS software.Results There were no significant differences in age,sex,body mass index(BMI),blood pressure(SBP,DBP)and blood type between the two groups(P>0.05).There were no significant differences in GLU,TG,HDL,LDL,APOA1,APOB,UA between the two groups(P>0.05).There was no significant difference in the previous medical history(essential hypertension,diabetes,cerebrovascular disease,peripheral atherosclerotic disease,gallbladder disease,fatty liver,obstructive apnea syndrome,thyroid disease,osteoarthritis),smoking history and drinking history between the two groups(P > 0.05).There were significant differences in total cholesterol and arteriosclerosis index between the two groups(P< 0.05).The results of multivariate logistic regression showed that the relative risk of LEL was 4.285 in the population with abnormal total cholesterol,and the corresponding 95% CI was(1.673,10.978).The relative risk of LEL in type B blood was 3.942,corresponding to 95% CI(1.313,11.832).And the above factors are all independent risk factors.Conclusion In obese patients,the total cholesterol and arteriosclerosis index in LEL group were higher than that in non-LEL group,abnormal total cholesterol and blood type B blood were independent risk factors for the development of the lumbar spinal epidural lipomatosis in obese patients.
