Objective:To raise the awareness about the pathogenesis, clinical manifestation, diagnosis and treatment of the rare syndrom:Spinal epidural lipomatosis(SEL).Method’.To Report four typical cases who visit our hospital from 2015.5 to 2016.3.Compare the general condition, clinical manifestation, imaging tests and treatment of all cases and telephone interviews about patients’ recovery were conducted after operation.The order is to analyze the clinical characteristics, diagnosis, distinguish diagnosis and treatment of this disease with reference from previous cases reported in literatures.Result:In the four cases, the average age is 64.5 with the sex are all male. All of them had no history of exogenous steroid therapy. Two of them are overweight with BMI>25, two of them are obesity with BMI>28. Three of the cases have got the disease more than 3 years. Three cases had typical intermittent claudication,while one had significant neurological deficit in bilateral lower limbs.The T1 phase of MR sagittal images of the four cases all showed quantity of high signal substance accumulation.Four cases were all treated with operation, and the method is transforaminal lumbar interbody fusion (TLIF),three cases obtain satisfactory results, while one patient don’t have full recovery.Conclusion:Most of the SEL patients without history of exogenous steroid therapy are elder male,weighing more than normal. The patients’ main clinical manifestation is intermittent claudication. MR examination is an effective means to diagnose SEL. There may be a connection between SEL and spine degeneration disorders. TLIF can be used as an effective method to treat SEL or the patients together with other spine degeneration disorders.
