Investigation About Anxiety/Depression Of Inpatients With Coronary Heart Disease

Objective The clinical data of hospitalized patients with coronary heart disease(CHD)diagnosed by coronary angiography(CAG)in the department of cardiology,general hospital of Ningxia medical university from December 2018 to October 2019 were analyzed to investigate the incidence of anxiety/depression status and related risk factors of CHD patients.Methods The psychological assessment of patients was completed by using the self-made scale,Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD),and the general clinical data of the patients were recorded and analyzed 1 to 2 days after CAG.Results 1.A total of 215 patients with CHD were included in this study,including 138males(64.19%)and 77 females(35.81%),with an average age of 61.77±10.37 years old,and92 elderly patients(≥65 years old)(42.79%).The incidence rates of anxiety,depression and anxiety combined with depression were 43.26%,33.49% and 26.05%,respectively.The incidence rates of females were all higher than that of males(63.6% vs 31.9%,51.9% vs23.2%,45.5% vs 15.2%,all P < 0.05).2.Single-factor correlation analysis showed that female,frequent insomnia and HAMD score were risk factors for anxiety state,frequent insomnia,mental tension and HAMA score were risk factors for depression state,and female,frequent insomnia and mental tension were risk factors for anxiety and depression state.Multivariate Logistic regression analysis and ROC curve showed that HAMD score ≥5.5 was an independent risk factor for anxiety.Poor economic status and HAMA score ≥8.5 were independent risk factors for depression.Frequent insomnia and mental stress are independentrisk factors for anxiety and depression.3.The sensitivity of HAMD score =5.5 was 75.27%,the specificity was 77.87%,and the youden index was 0.531.HAMA score =8.5 points showed a sensitivity of 73.61%,specificity of 83.22% and a youden index of 0.568 for the prediction of depressive state.Conclusion 1.The incidence of anxiety,depression and anxiety combined with depression was higher in this group of CHD patients,and the incidence of female patients was higher than that of male patients.2.Severe insomnia,mental tension and low income are related to anxiety and depression in CHD patients.HAMA score and HAMD score are of high value in predicting depression and anxiety of CHD patients.
