Objective:To understand the change level of TAT,PIC,TM,t-PAIC in trauma patients, and analyze the evaluation value of RAPT score combined with TAT,PIC,TM,t-PAIC in the occurrence of DVT in trauma patients. Methods:We collected a total of 133 trauma patients admitted to Bethune Hospital in Shanxi.Immediately after admission,RAPT scores and four new thrombus tests were performed(thrombomodulin(TM),thrombin antithrombin complex(TAT),tissue-type plasminogen activation inhibition complex(t-PAIC),Plasmin-antiplasmin complex(PIC)was divided into DVT group and non-DVT group based on the results of lower extremityvein ultrasound. The differences between the above indicators were compared, and the ROC curve was drawn to find the diagnostic cut-off point and diagnostic efficacy for the occurrence of DVT.Use Logistic regression and multiple diagnostic methods to improve the diagnostic value of DVT.Results:42 patients were diagnosed with DVT,and 91 patients were not diagnosed with DVT.There were significant differences in TAT and PIC between traumatic DVT patients and non-DVT patients. The concentrations of TAT and PIC in the DVT group were higher than those in the non-DVT group, and the differences were statistically significant,P?0.05.There was no significant difference between TM and t-PAIC and non-DVT,and the differences were not statistically significant,P>0.05;ROC curve analysis was performed on TAT and PIC.TAT: AUC 0.622,Cut-off value:3.9ng/ml,sensitivity:85.7%,and specificity : 35.2%.PIC:AUC0.727,Cut-off:1.034ug/ml,sensitivity :66.7%,and specificity:70.3%.When using Logistic regression combined diagnosis,it was found that the RAPT score and PIC were statistically significant(P?0.05)and could be used for combined diagnosis, but TAT was not statistically significant(P> 0.05),so they were excluded. The area under the ROC curve of the RAPT score combined with the PIC relative to the RAPT score alone and the PIC was AUC: 0.816,sensitivity 97.6%,specificity: 65.9%.Conclusion:The concentration of TAT and PIC in patients with traumatic DVT is significantly higher than that of non-DVT patients, and it has diagnostic value. The RAPT score combined with PIC is of higher value in the diagnosis of DVT in traumatic patients and can be used as an auxiliary diagnostic indicator in clinical work.
