Purpose: Disseminated intrascular coagulation(DIC)is a clinical syndrome characterized by bleeding and microcirculatory failure,which is induced on the basis of many potential diseases.The prognosis is poor and the mortality rate is high.Early intervention can improve the prognosis and reduce the mortality rate.At present,there is no gold standard for early diagnosis of DIC.Traditional blood coagulation measurement has certain limitations for early diagnosis of DIC.To explore the clinical significance of thrombin molecular marker in disseminated intrascular coagulation(DIC),which include Thrombin-antithrombin complex(TAT),thrombomodulin(TM),plasmin-antiplasmin inhibitor complex(?2-plamininhibitor-Early diagnostic value of plasmin complex(PIC)and tissue plasminogen activator inhibitor complex(t-PAIC).Methods: This study included 233 patients with bleeding tendency or thrombosis,according to the 2017 edition of the Chinese disseminated intravascular coagulation diagnostic score system(CDSS),which divided into dominant DIC group,pre-DIC group and non-overt DIC Group.We compare the differences in laboratory indicators between groups,and evaluate its value in the early diagnosis of DIC.The molecular markes were measured by qualitative chemiluminescence enzyme immunoassay performed on HISCL automated analyzers.All patients with suspected DIC were followed for 7 days to screen for development of overt-DIC.Results: In patients with overt-DIC,TAT [28.4(14.7-62.6)ng/Ml]?TM [17.8(12.1-69.1)TU/Ml)?PIC [3.980(1.207-9.525)ug/Ml]were significantly higher than those in nonovert DIC patients,t-PAIC was not difference between the overt-DIC and non-overt DIC groups.[40.8(25.5-93.9)ng/Ml]?TM [21.4(10.7-28.7)TU/Ml)?PIC [6.136(3.187-9.278)ug/Ml]?t PAIC [18.6(17.1-28.4)ng/Ml] levels were significantly higher in patients with pre-DIC than in non-DIC patients.The AUC shows that combined diagnostic curve of TAT,TM,PIC,and t-PAIC is 0.887,and AUC combined PT,APTT,and D-D is 0.933.Conclusions: The combination of TAT,TM,PIC and t-PAIC can improve the early diagnosis rate of DIC and provide the best timing for early clinical intervention and treatment.
