Objective : Through the analysis of pulmonary embolism patients and healthy controls between new markers before four thrombosis: thrombus regulatory proteins,thrombin-antithrombin complex,the original organization type fibrinolytic enzyme activator and its compounds,people fibrinolytic enzyme inhibitors-1 and conventional coagulation fibrinolytic enzyme complex resistance index: plasma prothrombin time,thrombin time,part of the activated clotting time live enzymes,and fibrinogen,the change of the value of D-dimer,explore new thrombus four markers in patients with pulmonary embolism in clinical testing value.Methods :In this paper,through collecting in June 2020 to December 2020 for the first time in the hospital during the period of the CTPA and diagnosis of pulmonary embolism patients who were not treated(n = 60,the median age: 70,range: 28-87 years old,male 28 cases,32 cases of female)as experimental group and the normal physical examination(n = 60,the median age: 62,range: 20-66 years old,25 cases of male,female 35 cases)as control group.All subjects met the inclusion and exclusion criteria and agreed to participate in the study.The citrate plasma of 120 subjects on the first day of admission was obtained.Immunofluorescence method were used to measure blood clots regulatory proteins(TM),the original tissue fibrinolytic enzyme compound activator and inhibitor-1(t PAI-C),fibrinolytic enzyme of fibrinolytic enzyme complex(PIC)and thrombin-antithrombin complex(TAT),with freezing method measuring part thrombin time(TT),activated clotting enzyme live time(APTT),fibrinogen(Fib)and plasma prothrombin time(PT),by immune turbidimetry D-dimer(D-D)values.The relationship between the above markers and pulmonary embolism was analyzed statistically.The significance test level was set as α=0.05.All data were analyzed by SPSS20.0 software.Results:1.In comparison of routine anticoagulant detection indexes PT,APTT,Fib,TT and D-D between the experimental group and the control group,except for TT,the levels of PT,APTT,Fib and D-D in the experimental group were all higher than those in the control group,and the difference was statistically significant(P<0.05).2.The levels of TM,PIC,TAT and t PAI-C markers of fibrinolytic molecules in the experimental group and the control group were all lower than those in the experimental group,and the differences were statistically significant(P<0.05).3.The area under the curve of TM,PIC,TAT,t TPAI-C and D-D for the diagnosis of thrombosis was 0.772,0.798,0.850,0.793 and 0.766,respectively,and the difference was statistically significant(P<0.05).ROC curve can be seen that the highest accuracy of TAT,area under the curve was 0.850(95% CI: 0.782 ~ 0.918),at this point the cutoff value is 4.700,the sensitivity and specific degree are 0.783 and 0.817 respectively,negative likelihood ratio(NLR)and positive likelihood ratio(PLR)are0.265 and 4.273 respectively,some index of 0.600,negative predictive value of 0.790,the positive predictive value(PPV)of 0.810.Conclusions: The average levels of TAT,PIC,TM,t PAI-C,PT,APTT,Fib and D-D in patients with pulmonary embolism were higher than those in normal population.TAT has the highest diagnostic efficacy in patients with pulmonary embolism,with a sensitivity of 78.3% and a specificity of 81.7%.From the clinical point of view,TAT,PIC,TM,t PAI-C and D-D are all valuable in the detection of pulmonary embolism.
