Objective: The aim of the study was to evaluate the changes in carotid elasticity and hemodynic before and after delivery in patients with severe preeclampsia and normal pregnancy using Ultrasound technology.Methods: The study involved Sixty-four patients with severe preeclampsia as case group,and Thirty-one age-and gestational weeks-matched normal controls.The subjects were completed the clinical datas and carotid ultrasound scans in prenatal and postpartum,Intimamedia thickness(IMT)? End-systolic diameter(Ds)? End-diastolic diameter(Dd)were measured by Quality intima-media thickness(QIMT)and M-mode ultrasound,then use of spectral Doppler ultrasound to obtain bilateral common carotid artery artery peak systolic velocity,(PSV),end diastolic velocity(,EDV),minimum diastolic velocity(MDV),ratio of peak systolic velocity to end-diastolic velocity(S / D),resistance index(RI),pulsatility index(PI),time average peak flow velocity(TAPV)and other blood flow parameters,calculate the pulse wave velocity(One-point pulse wave velocity,PWV?).Elastic parameters and hemodynamic parameters were statistically analyzed.Results:(1)Compared with the control group in prenatal,the parameters of prenatal SPE group such as Weight?BMI(Body Mass Index)?IMT?PWV? were increased(p<0.05),PSV decreased,the difference was statistically significant(all p <0.05).(2).The weight,BMI,IMT and PWV? of the postpartum SPE group increased compared with the NP group,the difference was statistically significant(all p <0.05),and there was no statistical difference in PSV,EDV,MDV,RI,PI,S / D,and TAPV(all p> 0.05).(3).Compared with the prenatal period in the SPE group,postpartum weight,BMI,PWV?,PI,S / D in the SPE group decreased(all p <0.05),PSV,EDV,MDV,and TAPV increased,and the difference was statistically significant(all p <0.05),there was no statistically significant difference in RI between the two groups(p> 0.05).(4)Compared with the NP group before birth,postpartum weight and BMI decreased in the NP group(all p <0.05),PWV? showed a decreasing trend,but the difference was not statistically significant(p = 0.50),MDV,EDV,TAPV increased,the difference was Statistical significance(all p <0.05).(5)Compared with the change value of PWV? before and after delivery in the NP group,the change value of PWV? before and after delivery in the SPE group showed an increasing trend,but the difference was not statistically significant(p = 0.135).(6)PWV? was positively correlated with age,body mass,body mass index,systolic blood pressure,diastolic blood pressure,Ds and Dd(Rall = 0.23,0.29,0.30,0.43,0.28,0.24,0.38;RSPE = 0.20,0.26,0.27,0.33,0.17,0.20,0.34,all P <0.05).Conclusion: The prenatal carotid artery elasticity has been damaged in patients with severe preeclampsia.Although the postpartum carotid artery elasticity has been significantly restored,it has not returned to the normal postpartum state of women with normal pregnancy history.Recovery is related.In addition,in patients with severe preeclampsia,the peak blood flow rate in the prenatal carotid artery systole is reduced,and the postpartum hemodynamics are restored to the same period as the normal postpartum pregnant women.
