The Changes Of Somatostain In The Plasma And Pain Threshold Of Diabetic Patients During Perioperative Period

Objective:To investigate the changes of somatostain in the plasma and pain threshold of diabetic patients during perioperative period.Methods:The 35 non-diabetic patiens(NDM group)and 35 patient with diabetes mellitus(DM group)was enrolled in the current study with lower limb fracture surgery.Before operation(T1),24 hours after operation(T2),blood samples from non-infusion elbow vein were collected.The plasma levels of somatostatin were measuered by enzyme-linked immunosorbent assay(ELISA).Thermal pain threshold,thermal pain tolerance threshold,ischemic pain threshold,ischemic pain tolerance threshold and tenderness threshold were measured by quantitive sensor testing before operation(T1)and 24 hours after operation(T2).Results:Compared with the NDM group,the level of somatostatin(SST)at T1 and T2 was significantly decreased in the DM group(P<0.05);compared with T1,the level of somatostatin(SST)in both NDM group and DM group at T2 was significantly increased(P<0.01).Compared with the NDM group,the thermal pain tolarence threshold,ischemic pain threshold and tenderness threshold were significantly increased at T1(P<0.05),the thermal pain threshold showed an increaseing trend at T1,while the thermal pain threshold and ischemic pain threshold showed an increaseing trend at T2.Compared withT1,the thermal pain tolerance threshold and the tenderness threshold at T2 were decreased(P<0.01);there was no significant change in pain threshold in NDM group at T2.Conclusion:The plasma level of somatostatin(SST)in diabetic patient is significantly increased during perioperative period,while the thermal tolerance threshold and tenderness threshold are significantly decreased,which may be related to surgical trauma,pain stimulus,stress and inflammatory response.However,the correlation between SST and the change of pain threshold is not clear.
