| Objective:To study the relationship between blood uric acid level and renal function in patients with type 2 diabetes(T2DM),and analyze the risk factors that affect their blood uric acid level,provide scientific basis for the prevention and treatment of diabetic nephropathy.Methods:A retrospective analysis of 749 T2 DM patients admitted to the Second Hospital of Shanxi Medical University from January 2014 to December 2015.The diagnosis of T2 DM is based on the diabetes diagnosis and typing criteria established by the World Health Organization in 1999.According to blood uric acid level,it was divided into high uric acid group(blood uric acid ≥305 umol / L)and normal uric acid group(blood uric acid <305 umol / L).Compare the clinical characteristics and laboratory data between the two groups of patients,analyze the correlation between blood uric acid levels and renal function,Multivariate Logistic regression analysis was used to determine the risk factors affecting blood uric acid levels.Results:There were significantly more males than females in the high uric acid group(P <0.05).The body weight,body mass index,total cholesterol,low density lipoprotein cholesterol,creatinine,cystatin C,α1 microglobulin,β2 microglobulin,and urine microalbumin were significantly higher in the high uric acid group than in the normal uric acid group(P<0.05),but the high-density lipoprotein cholesterol and glycated hemoglobin were significantly lower than those in the normal uric acid group(P <0.05).Pearson correlation analysis showed that blood uric acid levels were significantly positively correlated with serum creatinine,cystatin C,α1 microglobulin,β2 microglobulin,and urine microalbumin levels(r = 0.39,0.24,0.31,0.31,0.17,P all <0.05).Logistic regression analysis showed that weight,body mass index,total cholesterol were independent risk factors affecting blood uric acid levels(OR=1.59,95% CI:1.05~2.42,P<0.05;OR=2.21,95% CI:1.53~3.21,P<0.05;OR=1.96,95% CI:1.27~3.06,P<0.05).High-density lipoprotein cholesterol and glycated hemoglobin are negatively related factors that affect blood uric acid levels(OR=0.54,95% CI:0.38~0.77,P<0.05;OR=0.54,95% CI:0.37~0.78,P<0.05).Conclusion:T2DM patients’ blood uric acid levels are related to renal impairment.High-density lipoprotein cholesterol and glycated hemoglobin are negatively related factors that affect blood uric acid levels.Body weight,body mass index,and total cholesterol are independent risk factors that affect blood uric acid levels in T2 DM patients.For individuals with these characteristics,taking targeted interventions is of great significance for the prevention and treatment of diabetic nephropathy. |
