Objective:To investigate the significance of ambulatory blood pressure(ABP)in the diagnosis of hypertension in young and middle-aged patients with newly measured blood pressure.MethodsA total of 148 newly discovered patient with elevated blood pressure aged 18 to 64 years old were taken as the research subjects.ABP examination was performed to collect daytime,night-time and average 24-hour blood pressure for statistical analysis.The candidates were divided into groups A,B and C with reference to the grade of hypertension.Each group was divided into male and female groups according to gender,and divided into youth group(18-44 years old)and middle-aged group(45-64 years old)according to age.The diagnosis rate of ABP was compared.Results:1.87.8%(130/148 cases)of patients were diagnosed by ABP.The diagnosis rate of ABP was 78.4%,70.9% and 83.1% respectively according to the 24-hour blood pressure,daytime blood pressure and night blood pressure.The diagnostic rates of occasional blood pressure rises up to grade 1,2,and 3 were 82.1%,86.5%,and 96.8%.2.The diagnosis rates of ABP for males and females were 89.6%(86/96 cases)?84.6%(44/52 cases),with no significant difference.There was no difference in the ABP diagnosis rate between male and female patients according to the 24-hour blood pressure,daytime blood pressure and nocturnal blood pressure.The comparison results of each group showed that the diagnostic rate of reference daytime blood pressure value in group C was higher than that in women,with statistical significance(p<0.05),but the other differences were not statistically significant.The diagnostic rates of ABP in the young and middle-aged groups were 90.7%(97/107 cases)and 80.5%(33/41 cases),with no significant difference.The diagnostic rate ABP in the young group was higher than that in the middle-aged group only based on the 24-hour blood pressure and daytime blood pressure,the difference was statistically significant(p<0.05).And there was no difference in the diagnosis rate of ABP in two groups according to the nocturnal blood pressure.Among the groups A,B and C,the diagnostic rate of ABP referring to the daytime blood pressure in the youth group was higher than that in the middle-aged group,and the difference was statistically significant(p<0.05),but there was no significant difference in the diagnosis rate of ABP in the young and middleaged groups only referring to the 24-hour blood pressure and the night blood pressure.3.Correlation analysis: Systolic blood pressure(SBP)of casual blood pressure was correlated with SBP of ABP throughout the day(r=0.276,P=0.001),during the day(r=0.291,P<0.001),and at night(r=0.266,P=0.001).Diastolic blood pressure(DBP)of casual blood pressure was correlated diastolic blood pressure of ABP throughout the day(r=0.391,P<0.001),during the day(r=0.407,P<0.001),and at night(r=0.452,P<0.001).However,among the groups A,B and C,only the diastolic blood pressure measured by chance in group C had a certain correlation with the nocturnal diastolic blood pressure of ABP.Conclusion:Occasional blood pressure are influenced by many factors,ABP is more reliable diagnosis of hypertension method,can make patients get accurate diagnosis.There is no significant difference in the diagnosis rate between men and women.There is no difference in the diagnosis rate of ABP between young people and middle-aged people.As the occasional blood pressure level increases,the diagnosis rate of ABP increases.For young and middleaged patients with newly elevated blood pressure,ABP is recommended to confirm the diagnosis of hypertension.In addition to paying attention to 24-hour blood pressure and daytime blood pressure,more attention should be paid to nocturnal hypertension and more scientific treatment strategies should be developed.
