Objective:To evaluate the consistency of attainment rate and influence factors between office blood pressure measurement and ambulatory blood pressure monitoring.Methods: A total of 200 patients in our hospital from 2016.9 to 2017.9 were random collected,their general characteristics,office blood pressure,ambulatory blood pressure were took notes.Evaluate the classification and risk stratification among those with high office blood pressure,and antihypertensive drugs were used among those masked hypertension and sustained hypertension patients.The office blood pressure and ambulatory blood pressure measurement were carried out among those patients who were reviewed after one month,and then compared the reults of the two measurements.Result:In 200 cases,91 cases with normal clinic blood pressure,35 cases of them with masked hypertension with high 24 h ambulatory blood pressure;109 cases with high office blood pressure,among them 32 cases were hypertension stage 1,58 cases were hypertension stage 2,19 cases were hypertension stage 3.Among them there were 34 cases white coat hypertension patients,and 75 cases sustained hypertension patients.Among the reviewed masked hypertension or sustained hypertension patients,there were 76 cases' reviewed office blood pressure reached the standard,but there were 39 cases with high ambulatory blood pressure among them.34 cases 'reviewed office blood pressure did not reach the standard,but there were 9 cases with normal ambulatory blood pressure among them.The value of Kappa was 0.198 by Kappa test,indicating the attainment rate between office blood pressure and and ambulatory blood pressure is large different.Multifactor Logistic regression analysis showed that the difference of the attainment rate of blood pressure between office blood pressure and ambulatory blood pressure was maily related to the risk factors,such as age,smoking,BMI,COPD,AF and chronic kidney disease.Conclusion: Office blood pressure can not accurately display patient's blood pressure,because of white coat hypertension and masked hypertension,and there is a high false rate.This study suggest that age,smoking,BMI,COPD,AF and chronic kidney disease are maily factors which course the difference of the attainment rate of blood pressure between office blood pressure and ambulatory blood pressure.
