Background:Acute pancreatitis refers to the activation of pancreatin caused by a variety of causes,followed by a disease characterized by local inflammatory reaction of the pancreas with or without changes in the function of other organs.The morbidity of acute pancreatitis is increasing year by year,but the mortality,especially in severe acute pancreatitis,is still high,with many complications and heavy financial burden.There are many causes of acute pancreatitis,such as cholelithiasis,alcohol,hyperlipidemia and other causes,cholelithiasis is still the main cause of acute pancreatitis in China,while cholelithiasis and alcohol are the most common causes of acute pancreatitis in foreign countries.In recent years,with the improvement of living standard and the change of life style,the consumption of alcohol in China has increased year by year,and alcohol has gradually become another important cause of acute pancreatitis in China.At the same time,it has been found that the number of global drinks and the per capita consumption of alcohol are on the rise in varying degrees,and the alcohol constituent ratio of acute pancreatitis is considerable inconsistency in different countries and regions.Previous studies on the etiological analysis of acute pancreatitis vary in sample size,and there are also certain differences in the constituent ratio of alcoholic acute pancreatitis due to economy,culture,education,lifestyle,and other factors in domestic and abroad and in different time periods in China.Therefore,this article aims to search for relevant literatures on etiological analysis of acute pancreatitis in domestic and abroad for 20 years,and to use meta-analysis to discuss whether there are differences in the alcohol constituent ratio of acute pancreatitis in domestic and abroad for 20 years,and to summarize the change trend before 10 years and after 10 years in China,and so as to provide theoretical basis for the prevention of acute pancreatitis.Objective:To systematically evaluate the changing characteristics of alcohol constituent ratio in patients with acute pancreatitis in China for 20 years by meta-analysis,and to compare alcohol constituent ratio with acute pancreatitis in domestic and abroad,and so as to provide theoretical basis for the prevention of acute pancreatitis.Methods:Relevant literatures on etiological analysis of acute pancreatitis published in PubMed,Embase,Cochrane Library,CNKI,WanFang,and VIP databases between 1999 to 2018 were searched.The two investigators independently screened the literatures in strict accordance with established standards,discussed and reached an agreement with each other in case of disagreement,or settled through negotiation with the third person,and recorded researchs and reasons for the exclusion.After sorting out the literature,the quality evaluation and data extraction of the included literature were carried out,statistical analysis was conducted with Stata 12.0 software,and the forest map of each group was drawn.Meanwhile,publication bias analysis and sensitivity analysis were conducted.After sorting out the literature,the quality evaluation and data extraction of the included literature were carried out,statistical analysis and forest maps were performed by using stata software.Meanwhile,sensitivity analysis and publication bias analysis were also conducted.Results: 1.Literature retrieval results: initial literatures were gradually selected after weighting,reading titles,abstracts and full text.Finally,55 domestic and foreign literatures were brought into meta-analysis,involving 42930 patients.Among them,there were 26 literatures in China with 16786 AP patients,including 16 studies from 1999 to 2008 and 11 studies from 2009 to 2018.There were 29 literatures(30 studies)abroad,including 26144 AP patients.2.Statistical analysis results:(1)The alcohol constituent ratio of acute pancreatitis in China was 10.9%(95% confidence interval[CI]:8.9%~12.8%)for almost 20 years,and that in foreign countries was 30.3%(95%CI:27.2%~33.4%),the difference was statistically significant(P<0.001).(2)The alcohol constituent ratio of acute pancreatitis in China from 2009 to 2018 was 12.8%(95%CI:9.3%~16.3%),higher than 9.4% from 1999 to 2008(95%CI:7.2%~11.7%),the difference was statistically significant(P<0.001).3.Sensitivity analysis and the publication bias results:The sensitivity analysis indicated that the results in this meta-analysis were stable,and there was no significant publication bias.Conclusion:(1)The alcohol constituent ratio of acute pancreatitis in China is lower than other countries in the recent 20 years.(2)Compared with the previous 10 years,the alcohol constituent ratio of acute pancreatitis in China has increased in the last 10 years.
