Construction And Preliminary Application Of Health Care Scheme For Patients With Coronary Heart Disease

Objective:(1)The attitude and behavior of inpatients with coronary heart disease(CAD)in health care were investigated and the influencing factors were analyzed.(2)The specific types of participation in health care for patients with coronary heart disease were studied qualitatively.(3)Based on the theory of interactive patient participation in patient safety,a health care scheme for patients with coronary heart disease(CAD)was constructed by literature research,semi-structured interview and expert meeting.(4)Through the similar experimental study,the preliminary trial of care scheme was carried out and its influence on the knowledge,participation attitude,patient participation behavior and hospitalization satisfaction of patients with coronary heart disease(CAD)was discussed.Methods:(1)The attitude and behavior of 306 patients with coronary heart disease in cardiology department of 3 third-class general hospitals in Taiyuan were investigated by convenient sampling,and the influencing factors were analyzed to provide relevant basis for the later project construction(2)A semi-structured interview was conducted with 12 patients with coronary heart disease in a third A general hospital to understand the specific types of participation behavior and the needs of patients,so as to provide a theoretical basis for theconstruction of the later scheme.(3)Referring to the relevant literature at home and abroad and analyzing,combined with the previous semi-structure interview,the draft health care draft of patients with coronary heart disease(CAD)was initially formed.To organize the expert meeting of cardiology department and clinical nursing direction,revise and perfect the initial participation plan,and form the final version of coronary heart disease patients participating in health care program.(4)Seventy patients with coronary heart disease in cardiology department of a triple A general hospital were selected by convenient sampling and divided into groups according to the order of admission.34 patients in the control group were included in the traditional health education group,and 33 patients in the intervention group using the guidance manual.The general data questionnaire,the evaluation form of health knowledge of patients with coronary heart disease,the participation attitude scale of patients,the participation behavior scale of patients,and the satisfaction questionnaire of patients in hospital were used to investigate the patients before and after intervention.Results:(1)Inpatients with coronary heart disease have a good attitude to participate in health care,and their behavior needs to be improved.The attitude of patients with coronary heart disease to participate in health care was divided into(4.10�0.64)and(0.57)and(3.74�0.64)and(3.70�0.53)and(3.97�0.45)and(3.67�0.45).The results of multiple regression analysis showed that the main factors affecting the participation of patients with coronary heart disease in health care were mainly.Age,educational level,per capita family monthly income,whether there are other chronic diseases.(2)According to the theoretical framework of interactive patient participation in patient safety,the participation of patients with coronary heart disease in health careincludes 32 specific participating behaviors,including participation in adaptation to the environment,participation in understanding their own health status,participation in operation management,participation in cardiac rehabilitation knowledge,participation in discharge preparation,participation in secondary prevention,etc.;decision-making participation includes decision-making on medication,surgical modalities,cardiac rehabilitation,etc.;and appeal participation includes participation in hospital management promotion and advice.(3)Through literature analysis and qualitative interview results,the patient participation scheme was initially formed.Seven experts,including cardiology and clinical nursing,discussed and revised the preliminary participation plan,and formed the final version of the health care program for patients with coronary heart disease.(4)The baseline of the two groups was comparable between the control group and the intervention group.It was found that the scores of coronary heart disease health knowledge,patient participation attitude and patient participation behavior in the two groups after intervention were higher than those before intervention,except that the participation attitude of the control group was not statistically significant(P > 0.05),and the others were statistically significant(P < 0.05).After intervention,the scores of coronary heart disease health knowledge,participation attitude and participation behavior in the intervention group were significantly higher than those in the control group,and the satisfaction scores in the intervention group were significantly higher than those in the control group(P < 0.05).Conclusion:(1)Patients with coronary heart disease can participate in health care attitude,behavior needs to be improved.The main factors affecting the patient's participation are age,education,family monthly income,whether there are other chronic diseases,etc.(2)Patients with coronary heart disease participate in health care behavior with a variety of types,including mainly care participation,other also include decision-making participation and demand participation.(3)Based on cross-section investigation,qualitative research,literature analysis and expert meeting,a health care program for patients with coronary heart disease(CAD)was constructed,which covered patients from admission to discharge from admission to discharge.It could effectively improve the level of health knowledge,attitude,behavior and satisfaction of patients with coronary heart disease(CAD).In the future,it is necessary to further expand the sample size and carry out verification research.
