Background and ObjectiveAging is a relatively hidden and gradual change with age,while skin aging is the most external manifestation of aging itself,and it is easier to be detected.Aging skin not only shows a decline in normal physiological functions,but also affects people's appearance and quality of life.With the improvement of economic and living standards,the public demand for medical beauty is gradually increasing,and more and more scholars begin to explore safe,effective and rapid methods to improve facial rejuvenation.In recent years,it has been found that PRP can provide a large amount of concentrated growth factors,which can promote tissue regeneration,ulcer wound healing,vascular formation,and increase the survival rate of adipose tissue.Clinically,it has been widely reported that PRP has achieved good results in oral and maxillofacial surgery,orthopaedics,dermatology,and plastic surgery.PRP is obtained by centrifugation of autologous blood to obtain high concentration of platelet products.It contains a variety of growth factors,such as platelet-derived growth factor(PDGF),transformed progenitor factor(TGF),insulin-like growth factor(IGF),epidermal growth factor(EGF),and vascular endothelial growth factor(VEGF).Studies have reported that these growth factors synergistically promote fibroblast and collagen tissue regeneration,increase the number of capillaries,and improve skin aging.Different from simple tissue fillers such as hyaluronic acid and collagen,PRP is derived from autologous blood,with high histocompatibility,regeneration potential and better stability and durability.Earlier,dermatologists or plastic surgeons often observed the changes of skin conditions by naked eyes,and their evaluation criteria were mostly based on qualitative or quantitative test scales.However,these analysis scales were more complicated in clinical practice,and were too abstract for patients.Because the subtle changes of the skin can only be recognized by the naked eye,which has many limitations and will be affected by a series of subjective factors,people gradually began to explore more objective and accurate detection standards.With the progress of science and technology,people gradually find that medical imaging is very useful in the detection and diagnosis of skin,because it can put most of the local small skin conditions to observe,with higher accuracy,and can help clinicians to diagnose and monitor diseases.VISIA skin Imaging Systems(Fairfield,total of 8 detection items including spots,wrinkles,textures,pores,telangiectasia,brown spots,ultraviolet spots and porphyrins were identified,and the subjects were compared with corresponding skin types of the same age group in the database,and the percentile or score value was generated to evaluate the patient's skin status.Periorbital skin,as the weakest part of the whole body,is most significant in facial aging.There are many articles about PRP treatment for facial rejuvenation in domestic and foreign literatures,but relatively few reports on multiple skin characteristics in the periorbital region.This paper aims to evaluate the clinical efficacy of PRP in improving periorbital skin aging through clinical observation and objective data analysis.MethodsFrom May 2018 to August 2018,20 patients with periorbital skin aging,all female,aged 39-50 years,were selected as Fitzpatrick type III ~ IV skin(appendix table 1).Dr.Glogau skin aging classification(appendix 2)all patients with grade II were associated with various degrees of wrinkles,discoloration,facial skin laxity,and other manifestations of skin aging.All the 20 patients were thoroughly cleansed before the test,and rested for 30 min before the test(the indoor temperature was controlled at 22?,and the humidity was about 50%).Photos of the original skin at 45 degrees on the front,right and left oblique sides were collected by Canon digital camera in the photography room of the undergraduate room.The 5 indicators of wrinkles,texture,spots,pores and ultraviolet spots were detected by VISIA skin detector(USA).Then PRP was extracted and injected into the subcutaneous tissue around the orbit.After 30 days and 60 days of treatment,the same Angle photography and VISIA detection were performed again,and statistical treatment and effect evaluation were performed,and the improvement rate was calculated.At the same time,the satisfaction rate of patients before and after treatment was evaluated.Results(1)VISIA test results: for the improvement of wrinkles,texture and spots,the statistical test showed that the comparison between 30 days after treatment and 60 days before treatment was statistically significant(P < 0.05).There was statistically significant difference between the treatment of pores at 30 and 60 days after treatment and before treatment(P < 0.05),but there was no statistically significant difference between the treatment of pores at 60 and 30 days after treatment.For the improvement of UV spots,the comparison between 30 and 60 days after treatment and that before treatment showed no statistical significance(P > 0.05).(2)The improvement rate of wrinkles,texture and spots increased gradually 30 days and 60 days after treatment.The improvement rate of pores was slightly increased before and after treatment.Ultraviolet ray spot did not improve before and after treatment,aggravate instead.(3)At 30 and 60 days,after treatment,the patients made independent evaluation,and the overall satisfaction rate of the face was 70%,85% and 95%,respectively,which was higher than that before treatment,that is,the facial aging characteristics could be improved after PRP injection,and the subjective satisfaction could be improved.Conclusion(1)Single injection of autologous PRP can improve the tolerability of periorbital skin aging,significantly correct fine wrinkles,improve skin texture,and avoid excessive tissue fibrosis and subcutaneous nodules.(2)PRP can improve periorbital skin spots and discoloration.(3)PRP periorbital subcutaneous injection is easy to operate,with few adverse reactions,and can improve facial skin aging,which is worthy of clinical application and promotion.(4)After treatment the patients were satisfied with the overall improvement of periorbital skin aging.
