Research On The Status Quo And Influence Factors Of Uncertainty Of The Diseases Of Patients With Valvular Heart Diseases And Their Relatives

Objective To investigate the preoperative illness uncertainty status and related factors of patients with valvular diseases and family members to reduce illness uncertainty by earlier and guideline the intervention based on Risky factors from illness uncertainty.Method A total of 325 patients with valvular diseases planned for cardiovascular surgeries and their family members were enrolled from Nov.2018 to jun.2019.Material data was collected from the General Information Questionnaire for Patients and Families,the Mishel Uncertainty in Illness Scale for Adult and Family Member(Chinese version),the Social Support Rating Scale,the Simplified Coping Style Questionnaire.Descriptive analysis,Pearson correlation analysis,and multiple linear stepwise regression analysis were used for data analysis.Result1?The range of illness uncertainty from patients was from 37 to 132 with the median of 97.20�11.67.The unequal item was the highest in all four items as well as higher than overall illness uncertainty.2? The score of patients-originated coping styles was 30.90�3.00.The negative coping style was 10.17�2.12,while the positive copying style was 20.72�3.39.The total score of illness uncertainty was negatively correlated with �positive coping�(P<0.05),and positively correlated with �negative coping�(P<0.05).3? The total score of patient social supports was 42.79�4.26.Among which,the score of objective supports,12.98�2.82;subjective supports,23.57�2.47,utilization of supports6.24�1.32;.The total score of illness uncertainty was negatively correlated with subjective supports,objective supports,and social support total score.4? It was found by univariate analysis from patients that there was correlation between some variates and illness uncertainty including disease progression,complications,medical insurance type,residence with family members,occupations of family members,medical paid,understanding for diseases,education levels for both patients and family members,residence,family monthly income(P<0.05).5? It was found by regression analysis from patients that there was correlation between some variates and preoperative illness uncertainty of valve surgeries including education levels for both patients and family members,residence,family monthly income,occupations of family members,complications and negative copying style from patients and family members.6?The range of illness uncertainty from family members was from 67 to 116 with the median of 96.73�8.11.The unequal item was the highest.7? The score of family members-originated coping styles was 32.32�4.28.The negative coping style was 10.58�2.52,while the positive copying style was 21.74�3.94.The total score of illness uncertainty was negatively correlated with �positive coping�(P<0.05),and positively correlated with �negative coping�(P<0.05).8? The total score of family member social supports was 44.12�3.84.Among which,the score of objective supports,13.07�2.49;subjective supports,24.90�3.00;utilization of supports,6.14�1.14.The total score of illness uncertainty was negatively correlated with subjective supports,objective supports,and social support total score(P<0.05).9? It was found by univariate analysis from family members that there was correlation between some variates and illness uncertainty including disease progression,complications,medical insurance type,residence with family members,occupations of family members,medical paid,understanding for diseases,education levels for both patients and family members,residence,family monthly income(P<0.05).10? It was found by regression analysis from family members that there was correlation between some variates and preoperative illness uncertainty of valve surgeries including education levels for both patients and family members,residence,family monthly income,occupations of family members,complications and negative copying style from patients and family members.11? It was associated positively for illness uncertainty between patients and family members(P<0.05).Conclusion1?Preoperative illness uncertainty of patients before valves surgeries is significant with correlated factors including disease progression,complications,medical insurance type,residence with family members,occupations of family members,medical paid,understanding for diseases,education levels for both patients and family members,residence,family monthly income and negative copying style from patients and family members.2? Preoperative illness uncertainty of family members before valves surgeries is significant with correlated factors including disease progression,complications,medical insurance type,residence with family members,occupations of family members,medical paid,understanding for diseases,education levels for both patients and family members,residence,family monthly income and negative copying style from patients and family members.3?
