Purpose To research the distributive characteristics of demodex in eyelash and to ameliorate the method of microscopic loading examination about demodex in eyelash,and explore the relationship between the numbers and species of demodex and ocular surface disease index(OSDI)scores,and the loss of meibomian gland,providing theoretical basis for the study of the eye diseases related to demodex and the further improvement of microscopic examination about demodex in eyelash.Methods 100 eyes of 50 patients with scaly blepharitis were enrolled in this study,the patients visited the ophthalmologist of the second affiliated hospital of Fujian medical university from Feb.2018 to Mar.2019.Their names,genders,ages,OSDI,and the loss rate of meibomian gland were recorded.The row number eyelashes and number of eyelashes in the central part of upper and lower eyelid with a width of 3mm were calculated under the slit lamp.Two inner(palpebral conjunctiva side)and outer(skin side)rows of eyelashes were taken from each eyelid,4 for each eyelid,8 for each eye,and 16 for each person.The modified method of microscopic loading was adopted.Put the cedar oil on the glass slide to form a thin oil layer of 1cm×1cm.The eyelashes should be placed in the cedar oil completely.Record the species and numbers of demodex on upper and lower eyelid,inner and outer rows eyelash,and then measure the length of eyelash hair follicle root respectively.SPSS21.0 software was used for statistical analysis of the data.Results 1.A total of 50 patients(100 eyes)with scaly blepharitis were enrolled in this study.The detective rate of demodex in 100 eyes was 92%(92/100),the detective rate of adult demodex folliculorum was 89%(89/100),pre-adult demodex folliculorum was 45%(45/100),adult demodex brevis was 27%(27/100)and the pre-adult demodex brevis was not detected.In the detected demodex,the detective rate and the number of demodex brevis was the least,the pre-adult demodex folliculorum was second,the difference between adult demodex folliculorum,pre-adult demodex folliculorum and adult demodex brevis was statistically significant(P<0.05).2.No significant difference was noted between the rate of demodex detection in female (96%,48/50)and male(88%,44/50).The number of demodex in female(7.84±6.56/ eye)was higher than that in male(4.64±3.62/ eye),and the difference was statistically significant(P<0.05).3.The number of upper eyelid lashes,the number of eyelash rows,the length of eyelash hair follicle root,the total number of demodex,the number of adult demodex folliculorum and the number of pre-adult demodex folliculorum were all greater than those in lower eyelid,the detective rate of demodex(91%)was higher in upper eyelid than that in lower eyelid(74%),and the difference was statistically significant(P<0.05).No significant difference was noted in the number of adult demodex brevis between upper and lower eyelid(P>0.05).4.The total amount of demodex,the numbers of adult demodex brevis and the numbers of pre-adult demodex folliculorum in upper eyelid inner eyelash row were all greater than those in outer eyelash row,and the difference was statistically significant(P<0.05).No significant difference was noted in the numbers of adult demodex folliculorum and the detection rate of demodex between inner and outer eyelash(P>0.05).The total amount of demodex,the number of adult demodex brevis and the number of pre-adult demodex folliculorum in lower eyelid inner eyelash row were all greater than those in outer eyelash row,and the difference was statistically significant(P<0.05).No significant difference was noted in the numbers of adult demodex folliculorum and the detective rate of demodex between inner and outer eyelash(P>0.05).5.The total amount of demodex and the numbers of adult demodex brevis were strong positive correlated with the loss rate of meibomian gland(P<0.05).The total amount of demodex,the numbers of adult demodex folliculorum,the numbers of adult demodex brevis and the numbers of pre-adult demodex folliculorum were all positive correlated with OSDI scores(P<0.05).Conclusions The total amount of demodex was correlated with the loss of meibomian gland and ocular surface discomfort,while the numbers of adult demodex folliculorum were not significantly correlated with the loss of meibomian gland,the numbers of adult demodex brevis were positively correlated with the loss of meibomian gland.The most numbers of demodex mites were found in the inner eyelash of upper eyelid,so that the inner eyelash of the upper eyelid should be optimized in the microscopic examination.The modified method of microscopic loading is more convenient.
