Objective: 1.To investigate whether chemotherapy is a key factor affecting cognitive impairment in breast cancer patients and the influence of age,education,living style and mental life on cognitive impairment in breast cancer patients 2.To explore the differences in cognitive function(including subjective cognitive function and objective cognitive performance)between women with breast cancer and age matched healthy women.3.To study the correlation between subjective cognitive function and objective cognitive performance in breast cancer patients aged 25-75 years Method:A multicenter,cross-sectional study was designed to investigate the cognitive function,psychological status,and life-related abilities of participants who were 106 breast cancer received chemotherapy,and 100 breast cancer not received chemotherapy and 96 age-matched healthy women.The main method and content includes:(1)FACTCog scale was used to assess subject subjective cognitive function;(2)The Hopkins Verbal Learning Test(HVLT)was used to assess subjects' rapid and delayed memory;The Trail Making Test(TMT)was used to assess the subject's processing speed and executive function.(3)The Hospital Anxiety and Depression Scale(HADS)and Brief Symptom Inventory(BSI-53)were used to evaluate the psychological states of the subjects,such as anxiety and depression;(4)Health-Related Quality of Life(HR-QOL)scale was used to test the life status of the subjects.Analysis of variance was used to compare the differences of subjective cognitive function(fact-cog)between the three groups of subjects.The chi-square test was used to compare the processing speed and executive function among the three groups of subjects.Pearson correlation coefficient was used to analyze the impact of perceived cognitive impairment(PCI)on quality of life(QoL),others' evaluation(Oth),perceived cognitive ability(PCA),memory processing speed delay,memory psychology and life state.Regression analysis was used to determine the influence of non-therapeutic factors such as age,education,psychology and life status on cognitive function.Results:1.The subjective cognition and objective cognition performance of the two groups of breast cancer patients were worse than that of the healthy group.The PCI score of the breast cancer chemotherapy group was significantly higher than that of the healthy group(P=0.04).Similarly,the PCI score of the breast cancer non-chemotherapy group was significantly higher than that of the healthy group(P=0.032).However,there was no significant difference in subjective cognitive function between the two groups;Processing speed of two patients' groups was significantly slower than healthy control while there was no difference between two patients' groups.Both patient's groups had slower processing and executive function than the healthy group.2.Stratified regression analysis showed that GSI and self-management awareness could predict cognitive dysfunction after controlling for confounding factors.3.The total scores of FACT-Cog and PCI and Oth subscales were slightly correlated with memory,processing speed and executive function,with statistical significance(r=0.16~0.20,all P<0.01).PCA was slightly correlated with the processing speed and execution ability,which was of statistical significance(r=0.12~0.14,all P<0.05).Qol was slightly correlated with delayed memory,with statistical significance(r=0.12,all P<0.05).Conclusions:The results of this study found that chemotherapy may not be a critical factor in the associated cognitive changes in breast cancer patients.Furthermore,it proves that GSI and self-management awareness are risk factors which could have effect on cognitive function.The subjective cognitive function of FACT-Cog is correlated with memory,processing speed and executive function.In this study,it was found that the memory,executive function and processing speed of the patients all decreased to different degrees,which may prompt the application of clinical preventive and therapeutic measures.
