Objective:1 To construct an evaluation plan for the core competence training effect of community specialist nurses that based on the Kirkpatrick Model and provide a reference for scientifically and systematically evaluating the core competence training effect of community specialist nurses.2 To evaluate the training program effect of community specialist nurses,and discuss the impact of the training on the core competence,career development and work quality of community specialist nurses.Methods:1 The training program was implemented through the framework of community nurses core competence.According to the training courses system based on the core competence of community nurses constructed by the research team in the early stage,to formulate a standardization implementation plan for the core competence training of community nurses in Chongqing.2 Under the guidance of the Kirkpatrick Model,an evaluation planwas constructed with the core competency training of community specialist nurses by expert meeting method.The authoritative coefficient of experts was calculated by questionnaire survey.3 Quantitative research method was used to survey 64 participants who participated in community specialist nurse training.The evaluation index of trainee training effect includes the satisfaction survey,theoretical assessment,operation and clinical practice assessment,and community nurse core competence.Statistical analysis methods included Paired sample t test,rank-sum test and soon.4 The semi-structured interview method was used to interview the trainees and the managers and colleagues of their community health service centers.The Colaizzi 7 analysis method was used to analyze and refine the interview data.Results:1 Based on the preliminary research,a standardization implementation plan for the core competence training of community nurses in Chongqing was formulated.The content of the plan included training course,selection and training of teachers,training content,training time,and training format.2 Based on the Kirkpatrick Model,an evaluation program for the core competence training of community specialist nurses in Chongqing was constructed,including four levels: Reaction,Learning,Behavior,andResult.The Reaction evaluation indicators included students' satisfaction with the training courses,teachers,environment,and teaching methods;The Learning evaluation indicators included theoretical,operational,and clinical practice assessment;The behavior level evaluated the dynamic changes and differences of its core competencies with before,after,and after one year of training;Result evaluation indicators included the work performance,work ability and new technologies developed by the trainees after training.3 Effects of training3.1 Reaction level: Student satisfaction: The student satisfaction with the teaching atmosphere was(8.50±1.58)points,the satisfaction with the training course was(8.51±1.36)points,the satisfaction with the teacher was(8.88±1.34)Points,and the satisfaction with the teaching quality was(8.46±1.53)points;3.2 Learning level: Theoretical,operational and clinical practice ability assessment results: The theoretical scores of the trainees are increased from(67.19 ± 7.39)points before training to(78.45 ± 6.70)points after training,and the operational scores of the trainees are increased from(84.64±3.55)points before training to(91.64 ± 2.55)points after training.The differences were statistically significant(P < 0.05).The clinical practice was(91.50±1.66)points;3.3 Behavior level: the core competence of the trainees: The 64 trainees who participated in the training of community specialist nurses to survey their core competence with before training,at the end of the training,and after 1 years of training,which included with the ability of community nursing practice,communication and coordination,professional development legal ethics practical ability and critical thinking ability.The difference in scores was statistically significant(P <0.05);By comparing the core competences of each time node,before training the core competence scores are high at the end of training and after 1 years with training(P<0.05);The difference in core competence score at the end of training and after 1 year of training was not statistically significant(P>0.05);3.4 Result level: Using qualitative research methods: Semi-structured interviews were conducted on 10 trainees who participated in the training of community specialist nurses and 6 managers and 4 community nurses in their community health service centers.By Colaizzi 7 analysis method,a total of 4 themes were extracted:(1)The core competence of community specialist nurses was improved;(2)The professional identity was enhanced;(3)The desire to further study;(4)The training plan needs to be further improved.Conclusion:1 This study builds on the Kirkpatrick Models evaluation model as a theoretical to construct a Chongqing community specialist nurses' core competence training effectiveness evaluation plan,including the reaction,learning,behavior and result,which can evaluate community specialist nurses in a scientific,comprehensive and systematic manner training results.2 The training program of community specialist nurses in Chongqing can effectively enhance the core competence of community nurses,improve their working ability and level,and provide guarantee for further improving the quality and level of community nursing.
