Objective To improve the compliance of secondary preventive drugs in Enshi area by analyzing the current situation and influencing factors of secondary preventive drugs for ischemic stroke.Methods The patients with ischemic stroke diagnosed by the Department of Neurology in Enshi district from January 1,2018 to December 31,2018 were selected as subjects,and the cases were retrospectively analyzed according to the inclusion criteria and exclusion criteria of this study.Collect patients' basic information,past history,out-of-hospital general conditions and whether the patients still adhere to the doctor's advice to take secondary prophylactic drugs during the follow-up at 1 month,6 months and 12 months after discharge.The compliance of secondary prophylaxis drugs was counted at three follow-up time points,and the patients were divided into compliance group and non-compliance group whether they insisted on taking secondary prophylaxis drugs during the12-month follow-up period.Univariate and multivariate Logistic regression analysis was used to analyze the current situation and related factors of secondary prevention compliance of ischemic stroke in Enshi area.Results A total of 625 patients met the criteria of this study,including 183(29.28%)patients in compliance group and 442(70.72%)patients in non-compliance group.The overall compliance of secondary preventive drugs was 58.72%,39.20% and 29.28% at 1 month,6 months and 12 months,respectively.Univariate analysis between the two groups showed that there were significant differences in age,male,place of residence,occupation,education level,NIHSS score at admission,previous history of impaired glucose tolerance or diabetes,types of medicine taken after discharge and family care(P<0.05).The results of multivariate Logistic regression analysis of the two groups showed that age,place of residence in Enshi city and Xianfeng county,education level,previous history of impaired glucose tolerance or diabetes,3 and 4 kinds of drugs taken after discharge,and family companionship were all independent factors affecting the compliance of secondary preventive drugs at 12 months of age(P<0.05).ConclusionThe overall compliance of secondary prevention drugs for ischemic stroke in Enshi area is lower than the national average level,and the compliance decreases gradually with the passage of time of discharge;the patients who live in Enshi City and Xianfeng County,have a higher education level,have a history of abnormal glucose tolerance or diabetes,have family members for a long time after discharge,and take fewer kinds of drugs after discharge have better secondary prevention compliance.the older the patients,the worse the compliance.In order to effectively improve the compliance of secondary preventive drugs in this area,it is necessary to actively improve the medical staff,the medical system and the patients themselves.
