Objective: To retrospectively evaluate the curative effect of femoral neck fracture surgery in children,and to explore the causes of postoperative complications.Methods: Collected the cases of femoral neck fracture children treated in our hospital from January 2012 to December 2018 through the hospital electronic medical record system and imaging system,and the cases were divided into open and closed groups according to the reduction method.Gender,waiting time from injury to surgery,time spent on surgery,intraoperative blood loss,etc.were compared.Follow-up through X-rays,MRI,ECT and examination of hip movements in children to understand the efficacy and record whether there are complications,the efficacy evaluation using Ratliff evaluation criteria,complications include avascular necrosis of the femoral head,hip varus,infection,early closure of epiphyseal plate,etc.analyze the risk factors of major complications.Statistical analysis was performed using spss20 software,counting data was selected by chi-square test,measurement data was selected by independent sample t test,P<0.05 was considered statistically significant.Results: A total of 113 children with femoral neck fractures were treated surgically.According to the inclusion and exclusion criteria,a total of 72 patients were included.The patients were followed up for at least 12 months,with an average follow-up time of 21 months,and all fractures healed.A total of 10 complications occurred in the open reduction group,including 1 case of wound infection and sinus tract formation,1 case of hip varus,and 8 cases of femoral head necrosis;2 cases of closed reduction group had femoral head necrosis.Statistical analysis found that except the intraoperative bleeding volume and operation time in the open group was significantly higher than the closed group,there were no significant statistical differences between the open group and the closed group.The main complications AVN were analyzed,and it was found that the children's gender,age,fracture type,fracture reduction method,and the waiting time from injury to surgery had no significant effect on the occurrence of AVN,but the quality of fracture reduction had an effect on the occurrence of AVN.Conclusion: The surgical treatment of femoral neck fractures in children is acceptable,and the way of reduction has no effect on the clinical results of fractures.However,this fracture has a high complication rate.Femoral head necrosis is the main complication.The quality of fracture reduction has an impact on the occurrence of AVN.
