Analysis Of Distribution Of TCM Syndromes Type Of Chronic Kidney Disease In The Elderly In Xiamen Heshan Community

Objective:To investigate the epidemiology of Chronic Kidney Disease(CKD)in the elderly physical examination population in Heshan community of Xiamen,and to analyze the distribution of TCM Syndrome Types in the elderly with CKD,so as to obtain the prevalence of CKD in the elderly in this area,master the distribution of TCM Syndrome Types in the elderly in this area,and provide an important basis for the effective prevention and treatment of CKD in this community.Methods:A random and systematic sampling method was used to select 1000 elderly people in Heshan community of Xiamen from September 1,2018 to December 31,2019.According to the definition of CKD according to KDIGO guidelines,elderly people meeting the diagnostic criteria of CKD were screened out,and the prevalence of CKD was obtained.The CKD patients were identified by senior TCM physicians,Deputy director of Nephrology and above in our hospital,and the data were collected.The Excel table was entered,and SPSS 25.0software was used for statistical analysis and processing of the data to obtain the distribution of TCM syndromes in elderly CKD in Heshan Community,Xiamen.Results:(1)This study showed that the total number of elderly patients with CKD in xiamen heshan community was 202,the total prevalence was 20.2%,and the awareness rate was26.7%.There was no statistical difference between male and female(P > 0.05).Among them,there were 131 cases with decreased eGFR,the prevalence rate was 13.1%,and there was no statistical difference between male and female(P > 0.05).The prevalence of proteinuria was9.9% in 99 cases,and there was no statistical difference between male and female(P > 0.05).The prevalence of hematuria was 7.0% in 70 cases,and there was a statistical difference between male and female(P<0.05).(2)The distribution of TCM syndromes in 202 elderly patients with CKD in this study:the syndrome of deficiency of spleen and kidney yang is the main type,totaling 91 cases,accounting for 45% of TCM syndromes.The syndrome of dampness and heat was the dominant syndrome in the standard evidence,with a total of 85 cases,accounting for 42.1%.(3)In this study,the distribution of other TCM syndromes in 202 elderly CKD patients was as follows: spleen and kidney qi deficiency in 43 cases,liver and kidney Yin deficiency in 25 cases,qi and Yin deficiency in 24 cases,Yin and Yang deficiency in 19 cases,accounting for 21.3%,12.3%,12.0% and 9.4% of the total cases,respectively.There were 72 cases of wet turbidity syndrome,24 cases of blood stasis syndrome and 21 cases of water vapor syndrome,accounting for 35.6%,11.9% and 10.4% of the total cases,respectively.(4)In this study,the distribution of TCM syndromes in different age groups(65-74 years old,75-84 years old and over 85 years old)of elderly patients with CKD was different(P<0.05),but there was no gender difference(P>0.05).(5)In this study,the distribution of diabetes mellitus,hypercholesterolemia,hypertriglyceridemia and TCM syndrome types in elderly CKD patients were different(P<0.05).(6)In this study,there was a significant difference in the type distribution of BenxuSyndrome among the elderly patients with CKD under different standard evidence(P <0.05).Among them,deficiency of the spleen and kidney yang combined with dampness-heat syndrome,deficiency of the spleen and kidney qi combined with dampness-turbidity syndrome,deficiency of both qi and Yin combined with dampness-turbidity syndrome,deficiency of liver and kidney Yin combined with dampness-turbidity syndrome,deficiency of both yin and Yang combined with water-Qi syndrome were the most common.Conclusions:(1)The prevalence of CKD among the elderly in Heshan community of Xiamen was20.2%.(2)The TCM syndrome of Ben-deficiency in the elderly CKD population in Heshan Community in Xiamen is mainly spleen-kidney Yang deficiency,the standard syndrome is damp-heat syndrome,and the combined syndrome is mainly spleen-kidney Yang deficiency with Damp-heat syndrome.(3)The distribution of TCM syndromes in elderly CKD population in Heshan community of Xiamen is related to age,diabetes,hypercholesterolemia and hypertriglyceridemia.
