Objective: 1 To compare the differences in blood glucose fluctuations between patients with progressive cerebral infarction and those with non-progressive cerebral infarction.2 To analyze the degree of influence of blood glucose fluctuations on neurological deficits in patients with progressive cerebral infarction,and explore possible mechanisms.3 To through the analysis of blood glucose fluctuations in patients with hyperglycemia and acute cerebral infarction,we will provide guidance for the treatment of patients with hyperglycemia and acute cerebral infarction in future work and evaluate the outcome of patients with cerebral infarction.Materials and methods: To select from May 2016 to January 2018 the Department of Neurology in Minda Hospital Affiliated to Hubei University for Nationalities,select 40 cases of hyperglycemia complicated with progressive cerebral infarction admitted to,DMPIS group;Diabetes with non-progressive cerebral infarction 40 In the DMNPIS group,40 patients with cerebral infarction without blood glucose abnormalities were selected as the NDMPIS group.After collecting the general clinical data of patients,monitor the fast bed blood glucose monitoring data of all patients 8 times daily after admission(3 meals before meals,2 hours after meals,00:00 and 03:00 in the morning).Blood glucose fluctuations and correlation analysis,including SDBG,MAGE,PPGE,LAGE.At the same time,some clinically used indicators such as triglyceride,total cholesterol,low-density lipoprotein,high-density lipoprotein,glycosylated hemoglobin,fibrinogen,C-reactive protein(CRP),and homocysteine(HCY)were detected.All neurological deficits in patients with acute cerebral infarction were assessed using the National Institutes of Health Stroke Scale(NIHSS scale).The comparison of blood glucose fluctuations between the three groups and the correlation between general clinical data and serological indicators was performed using the Pearson correlation analysis.Pearson correlation analysiswas used to analyze the relationship between NIHSS scores and related factors in the groups I and II.The NIHSS score and NIHSS score were used as the dependent variables.Each relevant observation index was multivariate stepwise linear regression analysis.Results: 1 There was no significant difference in gender,age,BMI,HDL,TC,TG between the three groups(P>0.05).2 In the progressive cerebral infarction group,using the Pearson correlation analysis,the correlation between NIHSS score and each observation index,IMT,HCY,LAGE,MAGE,PPGE and NIHSS scores were positively correlated(P<0.05),and the NIHSS score was the cause.Variables,each of the above-mentioned correlation indicators are multivariate linear regression analysis of the independent variables into the final equation MAGE,HCY and HPPGE.The NIHSS score equation was NIHSS=0.941PPGE+0.08HCY+0.522MAGE-0.611.The increase of NIHSS score equation was: NIHSS=1.164PPGE+0.23HCY-3.97.(P<0.05)Conclusion: 1 Compared with patients without diabetes with cerebral infarction,blood glucose fluctuations in patients with abnormal blood glucose were significantly higher.2 In the two groups of patients with abnormal blood glucose and acute cerebral infarction,the mean blood glucose fluctuation amplitude,postprandial blood glucose fluctuation amplitude,and maximum blood glucose fluctuation amplitude in the progressive cerebral infarction group were greater.3 The degree of blood glucose fluctuations in patients with progressive cerebral infarction was positively correlated with changes in the NIHSS scores of patients.The greater the fluctuation in postprandial blood glucose,the higher the increase of NIHSS score.
