Status And Development Forecast Of Health Resources Allocation In Jiaozuo

Objective The level of health resource allocation objectively reflects the level of regional health services,this study with a view to providing development suggestions and data reference for the administrative departments analyzed the current situation and development of health resources allocation in Jiaozuo and proposes existing problems.Methods Literature retrieval and data analysis were used together.the literatures related to health resource allocation are searched,so as to sort out and analyze the problems of the health planning,the layout of medical institutions,the fairness of medical resources and the improvement of health service level.Data analysis in health resource allocation includes four parts: the general situation,fairness,proportionality and prediction.The general situation is mainly carried out through descriptive analysis.The fairness analysis is measured by Gini coefficient and Theil index under the two criteria of population and geographical area.The proportionality analysis is carried out through HRDI.As a extended model of GM model,ODDGM is used to forecast the development of health resources.Results In 2016,the number of health institutions per square kilometer in urban was higher than that in county,while the number of health institutions per thousand people in county was higher than that in urban.The distribution of health technicians in urban is better than that in County.The total number and proportion of health personnel in primary medical and health institutions are unreasonable.The structure of age,educational background and professional title of all kinds of health technicians is unreasonable.The growth of health beds is unstable,which is over-concentrated in urban areas and general hospitals.There is a gap between the bed-to-nurse ratio of 1:0.36 and the national planning standard of 2020.The number of equipment and total value of more than 10,000 yuan are mainly distributed in hospitals,of which the Jiefang and Shanyang are higher than the average level,the rest are lower than the average level,the Jiefang is the highest,and the Zhongzhan is the lowest.Based on Gini coefficient,in 2016,the order of fairness in allocating health resources according to population standard is: doctors,professional technicians,health beds,the number of equipment over 10,000 yuan,nurses and the total value of equipment over 10,000 yuan.The fairness is fairer,fairer,fairer,reasonable,reasonable and reasonable.The order of fairness of allocating health resources according to geographical area standard from high to low is: doctors,professional and technical personnel,health beds,the number of equipment over 10,000 yuan,the total value of equipment over 10,000 yuan,nurses.The fairness is as follows: reasonable,wide gap,wide gap,wide gap,wide gap.The allocation according to population standard is superior to geographical area standard.Based on Theil index,In 2016,the unfairness of allocation of health resources accessibility of medical and health services increased gradually.The density of health resources in Jiefang and Shanyang is the highest,Xiuwu is the worst,and the HRDI gap between urban and county areas is obvious.Horizontal comparison between cities shows that there is a big gap in HRDI between Jiaozuo and four first-tier cities(Beijing,Shanghai,Guangzhou,Shenzhen)and Zhengzhou,the capital of Henan Province.Conclusion To optimize the allocation of health resources in Jiaozuo City from the perspective of fairness and balance,it is necessary to strengthen the training of health manpower and discipline construction,enhance the attractiveness of posts,steadily promote the standardization of multi-point practice of doctors,improve the construction of multi-point practice system,and optimize and adjust the proportion of enrollment and training for various medical specialties.To scientifically plan the development of health resources,it is necessary to gradually change the regional health planning from population standard to geographical area standard;formulate preferential policies to encourage the employment of medical graduates at the primary health institutions so as to improve the structure of health personnel;establish and improve the data sharing system of health resources,carry out the precise allocation of health resources,and help the dynamic adjustment of health resources planning.
