Background and objective The occurrence and development of diabetes mellitus(DM)is closely related to both biological and psycho-social factors.At present,Internet + health management mode,especially We Chat platform,is widely used in chronic disease management.Based on the We Chat platform,multiple functions such as health education,medication guidance,diet guidance and disease monitoring can be provided to the target group,so as to help DM patients with better self-management.This study is mainly based on We Chat platform to guide the self-management of type 2 diabetes mellitus(T2DM)patients,and aims to evaluate the application value of this model in T2 DM patients’ management.Methods A total of 310 T2 DM patients admitted to our hospital from June 1,2016 to May 31,2017 were enrolled in the study.The baseline data of the patients and the questionnaire surveys of self-management behavior scale,health belief scale,anxiety self-rating scale and depression self-rating scale were conducted.310 patients with type 2 DM were divided into a follow-up group and a control group,with 155 patients in each group.Patients in the follow-up group were followed up by the follow-up management group at regular We Chat,while patients in the control group did not do any intervention.At the end of follow-up,biochemical indexes and questionnaire were compared between the two groups.Results The data of 310 T2 DM patients collected at the time of enrollment showed that the compliance rate of blood glucose control was only 23.9%,with poor self-management in blood glucose monitoring,foot care,special diet and exercise.After the follow-up,in terms of self-management behavior,patients in the follow-up group were higher than those in the control group in terms of total score and blood glucose monitoring,foot care,special diet and exercise,with statistically significant differences between the two groups(all P<0.05).In terms of health belief,in addition to the benefit dimension,the total score and other dimensions of patients in the follow-up group were higher than those in the control group,and the differences between the two groups were statistically significant(all P<0.05).The SAS and SDS scores in the follow-up group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The compliance rate of blood glucose control in the follow-up group was 43.8%,significantly higher than that in the control group(30.8%),and the difference between the two groups was statistically significant(P<0.05).The incidence of complications,readmission rate and mortality in the follow-up group were 26.7%,15.1% and 3.4%,respectively,which were lower than those in the control group(38.3%,26.3% and 8.3%).The differences between the two groups were statistically significant(P<0.05).Conclusions 1.Blood glucose monitoring,foot care,special diet and exercise may be the main reasons for poor self-management in T2 DM patients;2.The regular follow-up management based on We Chat platform is conducive to the control of patients’ blood glucose stability,the reduction of the incidence of complications,readmission rate and mortality,the improvement of patients’ health belief level and the establishment of better self-management ability.
