Objective:To explore the application effect of goal-oriented tech-nology of glycemic control in the management of gestational diabetes mellitus,and provide reference for the management of gestational diabetes mellitus.Methods:This study is an experimental research.We selected pre-gnant women diagnosed as gestational diabetes mellitus in the outpatient department of a top three-grade hospital from January to June 2018.According to the simple random sampling method from 1-96 of the number of lottery;odd was divided into experimental group,even was divided into the control group,each had 48 cases.The control group was given conventional blood glucose control program,and the experimental group adopted goal-oriented technology of glycemic control based on control group O Comparison of the two groups of patients included:?fasting blood sugar,postprandial 2h blood sugar in the following four weeks after the start of the intervention and glycosylated hemoglobin at the fourth week;?target blood glucose,body weight compliance rate during the intervention and glycosylated hemoglobin of admission;?pregnancy outcome(including maternal outcomes and neonatal outcomes).SPSS20.0 software was used for statistical analysis.Statistical descriptions of normal and variance homogeneous measures were performed using mean+standard deviation,and the mean comparisons were compared using two independent samples of the t test.The counting data is statistically described using the frequency(N)and percentage(%).According to the composition of the data,the chi-square test and the likelihood-chi-square test are used to compare the frequencies.When P<0.05,the difference was statistically significant.Results:1.Two groups of patients,postprandial 2h blood sugar of the experimental group in the following four weeks after the start of the intervention was lower than control group,which was statistically significant of the difference(p<0.05);there was no significant difference of the fasting blood glucose level after 4 weeks of intervention and glycosylated hemoglobin at the fourth week in two groups(p>0.05).2.During the intervention,the target blood glucose compliance rate and body weight compliance rate of the experimental group were higher than those of the control group(P<0.05).The GHb of experimental group was lower than the control group at admission,the difference was statistically significant(P<0.05).3.The total incidence of complications in pregnant women with pregnancy outcomes was lower in the experimental group(10.6%)than in the control group(40.4%),and the.difference was statistically significant(P<0.05).The incidence of cesarean section and placental abruption was lower in the experimental group than in the control group,and the difference was statistically significant(P<0.05).The incidence of hypoglycemia,polyhydramnios,premature rupture of membranes,placental abruption,puerperal infection,gestational hypertension,postpartum hemorrhage were no statistically significant(P>0.05)between the experimental group and the control group.4.The total incidence of neonatal complications in pregnancy outcomes was lower in the experimental group(4.3%)than in the control group(25.5%),and the difference was statistically significant(P<0.05).The incidence of macrosomia was lower in the experimental group than in the control group,and the difference was statistically significant(P<0.05).The incidence of neonatal asphyxia,neonatal hypoglycemia and low birth weight infants were not statistically significant(P>0.05)between the experimental group and the control group.Conclusion:Four weeks after the start of the intervention,the effect of postprandial 2h blood sugar of the patients with gestational diabetes by goal-oriented glycemic control technology is was better than the conventional method;the overall effect of glycemic control in patients with gestational diabetes is better than the conventional method;it can reduce the incidence of cesarean section and placental abruption in patients with gestational diabetes;it can reduce the occurrence of giant children in gestational diabetes.The incidence of pregnancy outcomes such as premature rupture of membranes and neonatal asphyxia in patients with gestational diabetes is not significant.
