ObjectivesTo understand the psychological status and care needs of preterm maternal separation,and to explore the impact of Focused Solution Approach on the physical and mental state of maternal puerperium during maternal and childbirth,in order to promote maternal physical and mental health.MethodsThis study is divided into two phases.The first stage: 13 cases of maternal and child separation of maternal premature were selected in the obstetrics of a third-grade hospital in Zhenjiang City from March to April 2018.Through one-on-one in-depth interviews to understand the psychological status and nursing needs of such women.The second stage: 90 cases of premature maternal and infant separation were randomly selected from the maternity wards of a third-grade hospital in Zhenjiang City,and were divided into the control group and the observation group according to the admission time.The control group was given routine nursing intervention in obstetrics,and the observation group received 6 times of focused solution mode nursing intervention on the basis of the control group.The evaluation indexes were Self-Rating Anxiety Scale,Self-Rating Depression Scale,General Self-Efficacy Scale,and World Health Organization quality of life measurement,to evaluate the maternal postpartum,discharge day,and postpartum 42 days of anxiety,depression,general self-efficacy,and quality of life and postpartum complications during the puerperium.Results1.Four themes were extracted from the interview results of maternal and child separation of maternal premature: The complicated mood of the baby when the baby was just admitted to hospital;Under great psychological pressure;Lack of psychological support;Lack of effective nursing intervention.2.On the day after delivery,the scores of maternal anxiety,depression,general self-efficacy,and physical,psychological,social,and environmental dimensions in the control group were 56.05±9.34,58.00±8.59,20.70±6.49,11.35±2.78,11.73±2.96,12.85±2.49,12.60±2.95 respectively.The scores of observation group were 55.58±9.97,57.55±8.74,21.30±6.12,11.20±3.09,11.53±2.85,12.23±2.49,12.28±2.81,the difference between the two groups was not statistically significant(P >0.05)and were comparable.3.On the day of discharge,the anxiety and depression scores of the control group were 52.98±9.84 and 55.08±8.43,respectively.The scores of the observation group were 47.90±8.42 and 55.08±8.43,respectively.The difference between the two groups was statistically significant(P<0.05),while the self-efficacy score was 24.08±5.79 in the control group and 26.15±6.10 in the observation group.There was no significant difference between the two groups(P>0.05).4.At 42 days postpartum,the four dimensions of physiological,psychological,social,and environmental dimensions of anxiety,depression,general self-efficacy,and quality of life in the control group were 48.68±8.43,51.30±8.95,26.15±5.53,and 12.33±2.71,12.50±2.68,13.75±2.23,14.03±2.29,respectively.While the observation group scores were 42.48±6.28,44.40±7.00,33.78±3.36,14.13±2.52,15.28±2.10,15.38±2.13,15.38±2.13,comparing the two groups have statistical significance(P<0.05),and two groups of maternal postpartum complications situation also has statistical significance(P<0.05).Conclusions1.Maternal and child separation of maternal premature has serious psychological problems,and effective nursing measures are urgently needed for intervention.2.Focusing on the solution mode of nursing intervention can reduce the level of anxiety,depression,improve their self-efficacy and quality of life,reduce the incidence of postpartum complications,and this method is simple and easy to implement.It is suggested that this kind of maternity ward for a wide range of clinical use,and actively promote to the routine care in order to improve the quality of care.
