Objective(1)To construct a cooperative comprehensive nursing intervention plan for the physical and mental health of parturients separated from mothers and infants.(2)To verify the effect of coordinated comprehensive nursing intervention on the phy sical and mental health of parturients with maternal-infant separation.Methods1.Construction of comprehensive nursing intervention program of physical and mental health of parturients with separation of mother and infantLiterature research:search the database at home and abroad to understand and analyze the cooperative comprehensive nursing intervention of physical and mental health.The design and current situation of the scheme provide reference for the construction of a coordinated comprehensive nursing intervention plan for the physical and mental health of parturients separated from mothers and infants.Group discussion:through the discussion of the research group,the contents,forms,time and personnel of the coordinated comprehensive nursing intervention plan for the physical and mental health of parturients with maternal and infant separation were determined,and the first draft of the comprehensive nursing intervention plan for the physical and mental health of parturients with maternal and infant separation was formed.Semi-structured interviews:semi-structured interviews were conducted on 5 parturients with maternal-infant separation,4 neonatal pediatric nurses and 3 obstetrical nurses,and the interview data were analyzed.the relevant contents were extracted to make a preliminary revision of the coordinated comprehensive nursing intervention program of physical and mental health of parturients with maternal-infant separation.Preliminary test:12 parturients with maternal-infant separation were selected for pre-test.Work against each other according to the feedback of the puerpera.The pre-plan is modified.Expert consultation:Consult 9 experts in pediatric nursing,obstetrical nursing,health education and so on.By calculating the experts' familiarity with the intervention program,the judgment score and the expert's authority coefficient,the enthusiasm and authority of the experts were evaluated.According to the suggestion of experts,the coordinated comprehensive nursing intervention program of physical and mental health of pregnant women with maternal and infant separation was modified to form the final draft of the intervention plan.2.Application of comprehensive nursing intervention program of physical and mental health of parturients with separation of mother and infant 74 parturients with maternal' and infant separation were selected from the obstetrical ward of Grade 3 A Maternal and Child Health Hospital in Henan Province.The first obstetrical ward was the intervention group(n=34),the second obstetrical ward(n=34)and the control group(n=32).On the basis of routine nursing,the intervention group was given cooperative comprehensive nursing intervention,and the control group received routine nursing.The resting heart rate,systolic blood pressure,fasting blood glucose,anxiety and depression were measured on the day of maternal-infant separation,3 days postpartum,the day of maternal-infant separation at the end of rehabilitation and discharge,and at the end of puerperal period 6 weeks after delivery Postpartum.The incidence of breast distension and lactation were calculated.The breast-feeding rate,lactation volume and patient satisfaction were counted during the reexamination 6 weeks after delivery.Data were analyzed with SPSS22.0.Relevant variables were preformed by descriptive statistic,chi-square test,t-test or Mann-Whitney U rank sum test,repeated measures of variance analysis or generalized estimation equations.It is statistically significant to P<0.05.Results1.Construction of intervention program(1)Literature research:search the research on the coordinated comprehensive nursing intervention of physical and mental health of parturients separated from mother and infant in Chinese and English,through the content analysis method to extract the intervention mode,intervention times,intervention purpose and content,grouping method and so on.(2)Expert consultation:9 questionnaires were sent out and 9 were recovered,with a recovery rate of 100%.The familiarity Cs of the expert group is 0.911,and the authority coefficient is 0.928.(3)Synergetic comprehensive nursing intervention program of physical and mental health of parturient with separation of mother and infant:after literature review,group discussion,semi-structured interview,pre-experiment and expert consultation,it was determined that the intervention duration of the intervention program was 6 weeks,a total of 6 times,face-to-face intervention for 5 times,and telephone follow-up for 1 time.The intervention program includes eight intervention themes:understanding the intensive care unit,protecting freshmen's health,mother's health,child safety,participating in guardianship,maternal love at the side,emotion regulation,protecting mother's health,protecting mother's health forever,never being separated,being grateful for you,your growth,being accompanied by me,embracing happiness,and blooming beautifully.2.Application of intervention programA total of 74 subjects were included in this study,including 8 cases of loss(10.81%of the wastage rate)and 66 cases of effective sample size,including 34 cases in the intervention group and 32 cases in the control group.(1)comparison of anxiety scores between the two groups after interventionAfter intervention,the results of the repeated measurement of variance analysis of the anxiety and depression scores showed that group effect,time effects,and interaction effects in anxiety and depression were statistically significant(P<0.01).The scores of anxiety and depression in intervention group were statistically significant at different time points.That is to say,the scores of anxiety and depression changed with time,and the change trend was different(P<0.01).(2)comparison of depression scores between the two groups after interventionThe analy sis of variance of repeated measurement of anxiety scores between the two groups after intervention showed that there were statistical differences in anxiety and depression scores among groups,time effect and interaction effect.The anxiety scores of the intervention group were significantly different at different time points,that is,the anxiety and depression scores of the intervention group changed with time and the change trend was different(P<0.01)?(3)comparison of physiological indexes of parturients with maternal-infant separation after interventionThe analysis of variance of repeated measurement of physiological indexes between the two groups after intervention showed that there was significant difference in the incidence of breast distension between the two groups(P<0.05),and there was no significant difference in the effect of lactation between the two groups(P>0.05).There was significant difference in time effect and interaction(P<0.01).(4)comparison of patient satisfaction after interventionThe statistical results of maternal nursing satisfaction in the two groups showed that compared with the control group,the intervention group had a higher percentage of satisfaction and total satisfaction,and a lower percentage of satisfaction and dissatisfaction.ConclusionThe main results are as follows:(1)the experts of the coordinated comprehensive nursing intervention program for the physical and mental health of mothers with separation of mothers and infants are highly authoritative and active.Well,experts agree that the intervention program is of clinical significance in promoting the physical and mental health of parturients with mother-to-child separation.(2)The coordinated comprehensive nursing intervention program of physical and mental health of parturients with separation of mothers and infants can effectively alleviate the depressive symptoms and anxiety of parturients with separation of mothers and infants,and has a positive effect on the stability of physiological indexes of parturients,and the effect is better than that of routine nursing mothers.
