Objective: We aimed to find the independent prognostic factors of the axial and pelvic chondrosarcoma, and to develop and validate a reliable nomogram for predicting the the 5-year and 10-year overall survival of patients with the axial and pelvic chondrosarcoma. Methods: A retrospective search of the Surveillance,Epidemiology and Final Results Database(US SEER Database)retrieved cases of chondrosarcoma that occurred in the axial(cranium and trunk bone)and pelvis between January 1988 and December 2015.Screening excludes cases that do not meet the study criteria.Epidemiological statistics were performed on all case data. The cases were divided into a training cohort and a validation cohort according to the random number table method.Univariate and multivariate Cox proportional hazard regression model analysis was performed on the two cohorts of data to determine the independent prognostic factors of all axial and pelvic chondrosarcoma(all P <0.05),and all independent prognostic factors were included in the construction of the nomogram, using R programming software to construct nomogram to predict the 5-year and 10-year overall survival(OS)of patients with axial and pelvic chondrosarcoma. The consistency of the C-indices and the calibration curve were used to evaluate the accuracy of the nomogram predictions through internal and external verification. Results: A total of 916 patients with axial and pelvic chondrosarcoma were found. After filtering,707 patients were eligible for the study. The patient's age, tumor size, tumor grade, number of different primary malignancies, tumor stage(degree of infiltration),and whether to receive surgery treatment. These six factors were all analyzed in the univariate and multivariate Cox proportional hazard regression model analysis. All the P <0.05.Based on these six independent prognostic factors,we have developed a effective nomogram that predicts the 5-year and 10-year overall survival of patients with the axial and pelvic chondrosarcoma.Internal verification results(C-indices value 0.782,95% CI: 0.754-0.810)and external verification results(C-indices value 0.772,95% CI: 0.721-0.823).The internal and external calibration diagrams(Figure 5)show a good agreement between the predictions from the nomogram we made and the actual observations. Conclusion: Age,tumor size, tumor grade, number of different primary malignant tumors, tumor stage and whether or not to undergo surgical treatment are independent prognostic factors for patients with axial and pelvic chondrosarcoma. In addition, the nomogram we constructed can accurately predict the 5-year and 10-year overall survival of patients with axial and pelvic chondrosarcoma, which will help provide patients with tumor prognosis consultations and help doctor make individualize surgeons and provide important reference for medical decisions.
