Objective:To explore the effect of ADOPT mode on lung rehabilitation exercise for newly diagnosed COPD patients,improve patient compliance,improve their lung function level and quality of life,and provide new ideas and reference for further development and improvement of pulmonary rehabilitation exercise mode and program for patients with COPD.Methods:?1?This experiment is a quasi-experimental study.100 samples of newly diagnosed COPD patients who met the inclusion criteria from January 2019 to September 2019 in a respiratory department of a tertiary hospital in Inner Mongolia Autonomous Region were selected using a convenient sampling method.Observation group and control group were 50 cases each.?2?Patients in the control group received conventional lung rehabilitation exercise models,which included introducing them to the disease-related knowledge of COPD,explaining the benefits of lung rehabilitation exercises,and guiding patients in breathing and exercise training.Patients in the observation group received ADOPT lung rehabilitation exercise.According to the five steps of this model,the patient's attitude to lung rehabilitation exercise was defined,their problems were defined,their open thinking was encouraged,and a plan was developed with the researchers and implemented.After discharge,the two groups of patients were followed up for 3 months,and the frequency of lung rehabilitation exercise,lung function?FEV1/FVC,FEV1%pred?,body mass index?BMI?,exercise endurance?6MWD?,dyspnea?m MRC?,and quality of life?CAT?were evaluated before intervention,1 month after discharge,and 3 months after discharge.SPSS25.0 was used to perform Chi-square Test,T-test,Rank sum test,and Repeated measures analysis of variance.Results:?1?A total of 93 patients completed the study,47 in the observation group and 46 in the control group.There was no significant difference in general information between the two groups of patients?P>0.05?,and they were comparable.?2?The frequency of lung rehabilitation exercise in the observation group was higher than that in the control group at 1 and 3 months after the intervention,and the difference was statistically significant?P<0.05?.Comparing the two groups,the patients in the observation group.The frequency of lung rehabilitation exercise at 1 month was higher than 1 month after discharge,and the difference was statistically significant?P<0.05?.The frequency of lung rehabilitation exercise at 3 months after discharge in the control group was lower than that of 1 month after discharge,and the difference was statistical Significance?P<0.05?.?3?There was no significant difference in pulmonary function levels?FEV1/FVC,FEV1%pred?between the two groups of patients before the intervention?P>0.05?.At 1and 3 months after discharge,the patients in the observation group had higher levels of lung function.It gradually increased before the intervention,but the patients in the control group showed a downward trend.The lung function level of the two groups was statistically significant?P<0.05?.The lung function of the patients in the observation group was 1 to 3 months after discharge.The level was better than the control group in the same period,and the difference was statistically significant?P<0.05?.The results of repeated measurement analysis of variance showed that the pulmonary function level was statistically significant in terms of different intervention times and interactions?P<0.05?.?4?There was no significant difference in BMI,6MWD,and m MRC between the discharge,the BMI and 6MWD in the observation group were significantly higher than before the intervention,and the m MRC was significantly decreased.The difference was statistically significant.Significance?P<0.05?,and the BMI and 6MWD in the control group were lower than those before intervention?P<0.05?,and there was no significant change in m MRC?P>0.05?.The results of in the control group,and the differences were statistically significant?P<0.05?;3 months after discharge,the total CAT score of the observation group patients.The scores of each item were lower than those of the control group,and the difference was statistically significant?P<0.05?.The two groups of patients were compared within the group.The observation group's total CAT scores at1 and 3 months after discharge and the scores of each item were compared with those before the intervention.By comparison,the difference was statistically significant?P<0.05?;the cough,sputum,sleep,and energy scores of the control group patients at the1st and 3th months after discharge were statistically significant compared with those before the intervention?P<0.05?.The results of repeated measurement analysis of variance showed that the total score of CAT was statistically significant in terms of different intervention time,intervention method and interaction?P<0.05?.Conclusion:?1?Applying the ADOPT mode to the lung rehabilitation exercise of newly diagnosed COPD patients can improve the compliance of patients with lung rehabilitation exercise,enhance the effect of rehabilitation exercise,improve the level of lung function,improve exercise endurance,reduce the degree of breathing difficulty,and then improve life quality.?2?The ADOPT model is effective for the management of lung rehabilitation exercise for newly diagnosed COPD patients,and can provide suggestions and ideas for clinical development and improvement of the pulmonary rehabilitation system.
