ObjectiveThrough the analysis of relevant data characteristics about newly confirmed HIV infected persons,the first CD4~+T lymphocyte count,and the analysis of CD4~+T lymphocytes and viral load after treatment in Hohhot 2017.1-2019.3,to understand the epidemiological characteristics of newly confirmed HIV infection in Hohhot,the baseline level of CD4~+T lymphocytes,the relationship between CD4~+T lymphocytes and viral load after treatment,and the evaluation effectsof antiviral treatment,for early detection of HIV infection in our city,reducing HIV transmission,strengthening follow-up testing,and providing timely resistance Virus treatment,improving the quality of life and prolonging the survival time of HIV-infected persons,to provide an effective reference for the prevention and control of AIDS in our city.MethodsBased on the data from the Hohhot Center for Disease Control and Prevention Center's AIDS Confirmation Laboratory from January 2017 to March 2019,the HIV-1 antibody was positive,and its corresponding CD4~+T lymphocyte count and viral load data were combined.The corresponding direct case database of the?Integrated AIDS Prevention and Control Data Information System?excluded previously HIV-1 antibody-positive infections,screened out newly confirmed HIV-infected persons,and SPSS19.0 was used to analyze the newly confirmed epidemiological data and laboratory test data(CD4~+T lymphocyte count and viral load)of HIV infected persons.The measurement data of non-normal distribution were statistically analyzed by non-parametric test,and expressed by M(P25,P75).Count data were expressed as rates or composition ratios.Univariate analysis was performed by X~2test.Multivariate analysis was performed by two-class logistic regression.P<0.05 was considered statistically significant.Spearman rank correlation was used to analyze the correlation between CD4~+T lymphocyte count and viral load,and a scatter plot was drawn.Results1.There are 365 newly confirmed HIV infected persons in Hohhot,with a male proportion as high as 93.15%.They are mainly Han nationality,30-59 years old,sent by hospital for examination,high school or technical secondary school,college or above,unmarried,domestic household and unemployed people,and are mainly sexually transmitted.2.Compared with 2017,the number of newly confirmed HIV-infected persons increased by 32.61%(45 cases)in 2018.The sample source and age difference between 2018 and 2017were statistically significant(P<0.05).Different gender,ethnicity,marriage,and culture There were no significant differences in the degree,inspection unit,occupation,domiciles,current address,transmission routes(P>0.05).3.In terms of the number of newly confirmed HIV infections in each quarter from 2017to 2019,the upward trend in the first quarter of 2017-2019;the upward trend in the second quarter to the third quarter of 2017-2018,and the decline in the fourth quarter.trend.4.There were 284 newly confirmed HIV-infected HIV patients tested for CD4~+T lymphocytes for the first time,and the follow-up ratio was 77.80%.According to the first CD4~+T lymphocyte count group,the differences among different ethnic groups,ages,transmission routes,and household registrations were statistically significant(P<0.05).There were no differences in gender,marriage,occupation,education level,inspection unit and current address Statistical significance(P>0.05).5.The maximum value of CD4~+T lymphocytes in newly confirmed HIV-infected patients for the first time was 1269,the minimum value was 2,and the median distribution was 345(209,476)cells/ul.According to the Mann-Whitney U test,the difference between the first CD4~+T lymphocyte count in 2017 and 2018 was statistically significant(P<0.05).The first CD4~+T lymphocyte count in 2018 was lower than in 2017.6.From the first CD4~+T lymphocyte count in each quarter from 2017 to 2019,Kruskal-Wallis H test showed that there was no significant difference in the first CD4~+T lymphocyte count in the first quarter from 2017 to 2019(p>0.05).There was no significant difference in CD4~+T lymphocyte count between the second quarter and the third quarter of 2017-2018(P>0.05).The difference in CD4~+T lymphocyte count for the first time in the fourth quarter was statistically significant(P<0.05),that is,the CD4~+T lymphocyte count for the first time in the fourth quarter of 2018 was lower than that in the fourth quarter of 2017.7.The CD4~+T lymphocyte test was carried out simultaneously in 153 patients before and after treatment,with a maximum value of 1150 and a minimum value of 5.The median distribution was 387(259,537)cells/ul.Before and after treatment,the CD4~+T lymphocyte counts in CD4<200 group and CD4?500 group showed significant difference(P<0.05),while there was no significant difference between 200?CD4<350 group and350?CD4<500 group(P>0.05).8.According to the CD4~+T lymphocyte count group after treatment,the differences among different ethnic groups,marriages,and examination units were statistically significant(P<0.05),and differences between different genders,ages,occupations,education levels,transmission routes,household and current address None were statistically significant(P>0.05).After two-class logistic regression analysis,the difference in CD4~+T lymphocyte counts after treatment with different marital status was statistically significant(P<0.05).9.Wilcoxon paired sign rank sum test was used,and the difference of CD4~+T lymphocyte count before and after treatment was statistically significant(P<0.05).104cases(67.97%)showed an increase in CD4~+T lymphocytes after treatment compared with that before treatment,and the immunologic effective rate was 67.97%.The difference of CD4~+T lymphocyte before and after treatment was statistically significant(P<0.05)in different sex and CD4~+T lymphocyte level before treatment.10.153 cases(41.92%)were tested for viral load after treatment.Among them,the maximum viral load is 106,the complete inhibition rate of virus is 91.50%,and the effective rate of treatment is 96.08%.11.Excluding 142 viral load(VL<100)data without specific values,the remaining 11cases were subjected to Spearman correlation analysis between HIV-1 viral load(Logl0)and CD4~+T lymphocyte.Spearman correlation coefficient rs=-0.618,P=0.043.It can be considered that there is a negative correlation between HIV-1 viral load and CD4~+T lymphocyte count after treatment.Conclusion1.The main population of newly confirmed HIV infection in Hohhot City is still men and men,and the self-protection awareness of this group should be strengthened to reduce the spread of AIDS.2.The number of newly confirmed HIV-infected people in Hohhot is an increasing trend,and most of them are infected in earlier years,and AIDS testing should be continued.3.Hohhot AIDS antiviral treatment of virological effect is better,and the immunological effect needs to be improved.This requires early identification of those who have been infected with unconfirmed HIV infection and timely antiviral treatment.
