A Study Of Control Objectives In Type 2 Diabetic Patients

Objective:Observe the changes of blood glucose,blood lipid,blood pressure(BP),body weight and other indicators of patients with type 2diabetes mellitus(T2DM)under different treatment durations,to evaluate the current situation of T2DM comprehensive target control and provide reference for T2DM stage treatment scheme,in order to further improve the compliance rate.Methods:A total of 566 patients with T2DM diagnosed and treated in the Endocrinology Department of the Third Hospital of Hebei Medical University from July 2018 to December 2019(all meeting the 1999 WHO diagnostic criteria for diabetes)were selected as the research subjects.According to the history and treatment,they were divided into 62 cases in the newly diagnosed and treated group(newly diagnosed and untreated or course of disease<3 months)(group No control,group NC),and continuous intervention group(course of disease?3 months)(group Control,group C).Group C was further divided into 3 subgroups according to the course of diabetes:83 cases in early intervention group(group C1)(3 months?5 years),119 cases in medium intervention group(group C2)(5?10 years),302 cases in long-term intervention group(group C3)(course?10 years),of which each group segment includes only the lower limit,excluding the upper limit.Analysis of each group of patients glycated hemoglobin(Hb A1c),total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density-lipid cholesterol(LDL-C),BP and body mass index(BMI)control levels and compliance,and compare them.Results:1.Comparison of control levels of indicators:1.1 Group NC and C:Hb A1c,TC,TG,HDL-C,LDL-C,systolic blood pressure,diastolic blood pressure and blood uric acid in group NC were 9.70(3.00)%,5.00(1.53)mmol/L,2.12(1.64)mmol/L,1.13(0.22)mmol/L,3.20±0.89mmol/L,39.50(18.50)mm-Hg,92.50mm Hg(13.75),26.52(6.04)kg/m~2 and 320.00(174.00)umol/L,respectively.Hb A1c,TC,TG,HDL-C,LDL-C,systolic blood pressure,diastolic blood pressure and blood uric acid in group C were 8.10(2.45)%,4.71(1.52)mmol/L,1.64(1.30)mmol/L,1.20(0.38)mmol/L,2.96±0.92mmol/L,134.00(20.00)mm Hg,87.00(14.00)mm Hg,26.35(4.29)kg/m~2and 348.00(125.00)umol/L.Compared with group NC,Hb A1c,TC,TG,LDL-C,and diastolic blood pressure were significantly decreased in group C,and the differences were statistically significant(P<0.05).1.2 Subgroups in group C:The Hb A1c of group C3 was 8.40(2.20)%,which was significantly higher than that of group C1's 7.70(2.10)%(P<0.05).The TG of group C3 was1.51(1.17)mmol/L,which was significantly lower than that of group C1's1.89(1.50)mmol/L and group C2's 1.17(0.32)mmol/L(P<0.05).The group C3diastolic blood pressure was 85.00(14.00)mm Hg,which was significantly lower than the group C2's 90.00(14.00)mm Hg,and the differences were statistically significant(P<0.05).The uric acid in the group C3 was338.00(126.75)umol/L,which was significantly lower than the 364.00(132.75)umol/L in the group C2(P<0.05).The C-peptide of group C3 was 2.06(1.33)ng/ml,which was significantly lower than that of group C1's 2.65(1.42)ng/ml and group C2's 2.77(1.70)ng/ml.2.Compliance with control indicators:2.1Group NC and C:The group C patients'Hb A1c,TG,HDL-C,and BP compliance rates were 24.2%,53.4%,67.1%and 16.3%,which were significantly higher than the group NC's 12.9%,33.9%,53.2%and 4.8%,and the differences were statistically significant(P<0.05).2.2 Subgroups in group C:Hb A1c compliance rate of patients in group C1 and C2 was 33.7%and31.9%,respectively,which was significantly higher than 18.5%in group C3,and the difference was statistically significant(P<0.05).The group C3's TG compliance rate was 58.9%,which was significantly higher than the group C2's 45.4%,and the difference was statistically significant(P<0.05).The BP compliance rate of patients in the group C1 was 22.9%,which was significantly higher than the 10.1%in the group C2,and the difference was statistically significant(P<0.05).Conclusions:T2DM patients generally have metabolic disorders such as blood glucose,blood lipids,blood pressure,body weight and uric acid,and the the compliance is low after treatment.Based on the whole course of management,intensive weight,blood lipids(TC,LDL-C)intervention in the early course,focus on blood pressure standards in medium-term attention,long-term prevention of reverse hyperglycemia.
