ObjectiveThis study is based on the relevant theories of art therapy,through the application of mandala drawing therapy for psychological intervention in patients with recurrent spontaneous abortion,to explore the effect of intervention on anxiety,depression,pregnancy stress,and sleep quality in early pregnancy of patients with recurrent spontaneous abortion,and the effect of intervention on the changes in progesterone levels and the continued pregnancy at 12 weeks of gestation in patients with recurrent spontaneous abortion,and compare it with the effect of general psychotherapy,provide a reference for the psychological intervention study of anxiety and depression in patients with recurrent spontaneous abortion,further promote the application of painting intervention in the mental health care of clinical patients.MethodsUsing randomized controlled clinical trials,a total of 82 re-pregnant RSA patients who were hospitalized in the gynecology department of a top three hospital in Chengdu from March 2019 to December 2019 and met the inclusion criteria were selected.The patients were included in the test group(41 cases)and the control group(41 cases)by random number table method.Before the intervention experiment,a preliminary research plan is formed through the literature research method,and the final research plan is determined after preliminary experiments and consultation with expert opinions.At the beginning of the intervention,the general information questionnaire,Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),and Pregnancy Pressure Scale(PPS),Pittsburgh Sleep Quality Index(PSQI)were used to collect baseline data such as general data,anxiety level,depression level,pregnancy stress score,sleep quality score.The control group took general psychological treatment for psychological intervention,and the experimental group took mandala drawing therapy for psychological intervention.Intervention on the second weekend to evaluate the level of anxiety and depression for the second time,intervention on the fourth weekend to evaluate the level of anxiety and depression for the third time,and evaluate pregnancy stress and sleep quality.After intervention,the progesterone levels of the two groups of patients and the continued pregnancy at 12 weeks of gestation were compared.ResultsA total of 71 cases completed the study,including 36 cases in the experimental group(excluding 2 cases,3 cases dropped),and 35 cases in the control group(excluding 2 cases,4 cases dropped).The specific research results are as follows: 1.Anxiety levelRepeated measures analysis of variance showed that the difference between intergroup effect,time effect and interaction effect of the anxiety scores of the two groups of patients was statistically significant.The results of the comparison between the groups showed that there was no statistically significant difference in the anxiety scores between the two groups before the intervention(P>0.05);the anxiety scores of the experimental group were lower than those of the control group on the second weekend of the intervention and the fourth weekend of the intervention(P<0.05).The results of the comparison within the group showed that the anxiety score of the experimental group showed a downward trend with the intervention,and there was a statistically significant difference between the anxiety scores before the intervention,the second weekend of the intervention,and the fourth weekend of the intervention(P<0.05);the anxiety score of the control group showed a downward trend with the intervention,there was a statistically significant difference in the anxiety scores between the preintervention and the second weekend of the intervention and the pre-intervention and the fourth weekend of the intervention(P<0.05),there was no statistically significant difference in the anxiety score between the second weekend of intervention and the fourth weekend of intervention(P>0.05).2.Depression levelRepeated measures analysis of variance showed that the difference between intergroup effect,time effect and interaction effect of the depression scores of the two groups of patients was statistically significant.The comparison between the groups showed that there was no statistically significant difference in depression scores between the two groups before the intervention and the second weekend of the intervention(P>0.05);At the fourth weekend of intervention,the depression score of the experimental group was lower than that of the control group,the difference was statistically significant(P<0.05).The results of the comparison within the group showed that the depression score of the experimental group showed a downward trend with the intervention,and there was a statistically significant difference in the scores of depression before the intervention and the fourth weekend of the intervention,the second weekend of the intervention and the fourth weekend of the intervention(P<0.05);there was no statistically significant difference between the control group before intervention,the second weekend of intervention,and the fourth weekend of intervention(P>0.05).3.Pregnancy stressThe results of the comparison between the groups show that there is no statistically significant difference between the total score of PPS and the score of each factor between the experimental group and the control group after intervention(P>0.05).The comparison results within the group showed that after the intervention in the experimental group,the total PPS scores,factor 1,factor 2,and factor 3 scores were lower than before the intervention,the difference was statistically significant(P<0.05),there is no statistically significant difference in other items(P>0.05);there was no significant difference in the total PPS scores and scores of various factors between the control group before and after intervention(P>0.05).4.Sleep qualityThe results of the comparison between the groups showed that the total scores of PSQI,sleep quality,sleep latency,sleep duration,and sleep efficiency of the experimental group were lower than those of the control group,and the difference was statistically significant(P<0.05);there were no significant differences in the scores of sleep disturbance and dysfunction between two groups(P>0.05).The results of the comparison within the group showed that,except for the day dysfunction factor,the total score and other factors scores of the experimental group were lower than before the intervention,and the difference was statistically significant(P<0.05);the total PSQI score of the control group before and after intervention was statistically significant(P<0.05),comparing the scores of other factors,the difference was not statistically significant(P>0.05).5.Progesterone levelThe results showed that there was no statistically significant difference in the rise of progesterone between the two groups after intervention(P>0.05).6.Continued pregnancy at 12 weeks of pregnancyThe results showed that the rate of continued pregnancy at 12 weeks of pregnancy in the experimental group was 86.1%,and the control group was 82.9%.There was no statistically significant difference between the two groups(P>0.05).Conclusions1.Mandala drawing therapy can relieve the anxiety and depression level of patients with recurrent spontaneous abortion in early pregnancy.2.Mandala drawing therapy can relieve the pregnancy pressure of patients with recurrent spontaneous abortion in early pregnancy to a certain extent.3.Mandala drawing therapy can improve the quality of sleep of patients with recurrent spontaneous abortion in early pregnancy.4.Mandala drawing therapy has no obvious effect on progesterone growth in early pregnancy in patients with recurrent spontaneous abortion.5.The effect of mandala drawing therapy on the continued pregnancy at 12 weeks of pregnancy in patients with recurrent spontaneous abortion need further study.6.The psychological intervention of mandala painting based on the related theories of art therapy has the characteristics of easy implementation,low investment cost,easy to master,etc.It has strong flexibility and applicability,and can be popularized and applied in the psychological intervention of clinical patients.
