| Objectives This study aimed to explore the level of psychosocial adaptation and its influencing factors in patients with type 2 diabetes,so as to provide theoretical basis for the establishment of health education,psychological care and nursing measures for type 2 diabetic patients in clinical nursing work.Methods 508 patients with type 2 diabetes were selected from September 2018 to August 2019 in two Third-class hospitals in Datong.The general status questionnaire,Self-report Psychosocial Adjustment to Illness Scale(PAIS-SR),Family APGAP Index,Diabetes Distress Scale,Summary of Diabetes Self-care Activities and Stress Scale were used as research tools to analyze the psychosocial adaptation level and its influencing factors of patients with type 2 diabetes.SPSS22.0 statistical software was used to analyze the data,such as Independent sample T test,one-way analysis of variance,Pearson correlation analysis and multiple linear regression analysis.Amos 24.0 statistical software was used to construct structural equations.Results 1 The psychosocial adaptation score of 508 patients with type 2 diabetes was(55.37±17.89),and the item average scores in each dimension ranked from high to low were environment(1.49±0.76),work ability(1.43±0.69),health care(1.34±0.54),sexual ability(1.20±0.57),psychological status(1.08±0.58),communication status(1.03±0.73),and family environment(0.91±0.48).2 Single factor analysis showed that gender,age,marriage,occupation,education level,Monthly household income,place of residence,mode of living,medical payment,smoking,drinking,exercise,sleep,number of hospitalizations,course of disease,glycosylated hemoglobin(Hb A1c),fasting blood glucose,regular blood glucose monitoring,diabetic retinopathy,diabetic nephropathy,diabetic cardiovascular disease,diabetic cerebrovascular disease,diabetic peripheral neuropathy,diabetic foot,infection,family care,diabetes distress,self-management and stress may be the influencing factors of psychosocial adaptation in patients with type 2 diabetes(P<0.05).3 Multiple linear regression analysis showed that age,exercise,Hb A1 c,diabetic retinopathy,diabetic foot,family care,diabetes distress,selfmanagement and stress entered the regression equation,which were the influencing factors of psychosocial adaptation in patients with type 2 diabetes(P<0.05).4 Pearson correlation analysis and structural equation model results showed that family care and self-management were negatively correlated with psychosocial adaptation(P<0.01),the correlation coefficient were(r=-0.470,-0.366).Diabetes distress and stress were positively correlated with psychosocial adaptation(P<0.01),the correlation coefficient were(r=0.530,0.414).Diabetes distress,self-management and stress played partial mediating roles between family care and psychosocial adaptation.Diabetes distress and self-management played a chain-mediating role between family care and psychosocial adaptation.Diabetes distress and stress played a chain-mediating role between family care and psychosocial adaptation too.Conclusions The psychosocial adaptation of 508 patients with type 2 diabetes is at the median level.Age,exercise,Hb A1 c,diabetic retinopathy,diabetic foot,family care,diabetes distress,self-management and stress are the influencing factors of psychosocial adaptation in patients with type 2 diabetes.The better the family care and selfmanagement,the better the psychosocial adaptation of patients with type 2 diabetes.The greater the diabetes distress and stress,the worse the psychosocial adaptation of patients.Family care not only directly affects psychosocial adaptation,but also indirectly affects the psychosocial adaptation of patients with type 2 diabetes through the multiple mediating effects of diabetes distress,self-management and stress.Figure 3,Table 18,Reference 152... |
