Objective: To explore the safety and therapeutic effect of local infiltration anesthesia and lumbar epidural anesthesia in tension-free inguinal hernia repair in the elderly so as to provide reference for clinical practice.Methods: A total of 161 cases of tension-free inguinal hernia repair presented in the first hospital of Hebei Medical University from September 2017 to September 2019 were analyzed retrospectively.The patients were divided into local infiltration anesthesia group(n=90)and lumbago and epidural anesthesia group(n=71).Differences in preoperative general index,intraoperative anesthesia plus operation time,intraoperative blood loss,as well as the feeding time,length of stay,hospitalization costs and complications after operation were observed and compared between the two groups.Results: There was no significant difference between in preoperative general index,intraoperative blood loss,incision infection,scrotal hematoma and edema,and postoperative VAS pain score between the two groups(P >0.05),and no statistical significance was observed.The local infiltration anesthesia group was lower than lumbar epidural anesthesia group in terms of anesthesia plus operation time,postoperative feeding and drinking time,postoperative hospitalization time,postoperative hospitalization cost,postoperative hypotension,nausea,vomiting,and urinary retention(P<0.05),and the difference was statistically significant.Conclusions: In a comparative study of the clinical value of tension-free inguinal hernia repair in the elderly,compared with lumbar epidural anesthesia,tension-free inguinal hernia repair under local infiltration anesthesia can reduce hospitalization costs,shorten hospitalization time and reduce postoperative complications,local infiltration anesthesia is a more economical,simpler and safer anesthesia method,which is worthy of wide promotion in clinical application.
