Objective: To compare the clinical efficacy between endoscopic thyroidectomy via breast approach with traditional thyroidectomy in the treatment of benign thyroid disease.Methods: In this study,among the patients with gland surgery performed by the same one surgeon in department of breast and thyroid surgery,Hebei General Hospital from July 2014 to December 2019,53 patients were selected according to the inclusion and exclusion criteria,as the endoscopic group.According to the same inclusion and exclusion criteria,53 patients were randomly selected from the traditional open thyroid surgery performed by the same surgeon at the same time,as the traditional group.We collected the clinical data of the two groups of patients,compared the general data of the two groups,including gender,age,nodule diameter,pathological type,unilateral and bilateral surgery,to determine whether the difference between the general data of the two groups was statistically significant,and whether the two groups were comparable;then we compared the observation indexes of the two groups: operation time,intraoperative blood loss,postoperative drainage volume,drainage time,postoperative hospital stay,postoperative pain medication requirements,and the incidence of various complications of the operation,to determine whether the difference between the observation indexes of the two groups was statistically significant.We used the SPSS 21.0statistical software to analyze the data,P<0.05 was considered statistically significant.Results:1.There was no statistically significant difference between the two groups in gender,age,nodule diameter,pathological type,and single and bilateral thyroid surgery(P>0.05),and the two groups were comparable.2.In the endoscopic group,3 cases were converted to open surgery(2cases due to intraoperative hemorrhage and 1 case due to neck skin burns),and the remaining 50 cases were completed by endoscopic surgery;2 cases had early skin burns(1 case in the neck,1 case in the chest).The operation time,postoperative drainage volume,and drainage time of the endoscopic group were more than that of the traditional group,and the difference was statistically significant(P<0.05).There were no statistically significant differences in the intraoperative blood loss,postoperative hospital stay,postoperative pain medication requirements,various operations and incidence of complications(hypoparathyroidism,pathology see parathyroid tissue,recurrent laryngeal nerve injury,superior laryngeal nerve injury,esophageal injury,tracheal injury,vascular injury,and subcutaneous effusion)(P>0.05).Conclusion: Endoscopic thyroidectomy via breast approach for benign thyroid diseases is not significantly different from the traditional group on the safety,feasibility;however,the operation time,the drainage volume and the drainage time is more than traditional surgery.
