Application Of Care Bundles In Preventing Vascular Crisis After Replantation Of Severed Finger

Objective: To explore the application effect of care bundles in preventing vascular crisis after replantation of severed finger,so as to provide basis and reference for reducing the incidence of vascular crisis,increasing the survival rate of replantation finger.Methods:Selection in January 2017-June 2019 a third class a hospital treated fingers from 120 patients with middle and line wounded finger as the object,in 2018,from April to June 30,2019,60 patients were treated for prospective observational group(care bundles group)in January 2017 to March 31,2018,60 patients were treated for historical control group in the control group given conventional nursing care of the wounded finger,including postoperative blood supply,health education,limb to keep warm,heat lamp,painkillers prescribed application,etc Observation group in the control group based on the combination of care bundles,including setting up care bundles team,to have evidence-based foundation of cluster scheme and the implementation of pain assessment,according to the pain score according to the three ladder principle used painkillers,observe the replantation refers to peripheral blood supply in time to upload photos and We Chat group,form management and so on to compare two groups of patients pain digital scoring method(NRS)score,the incidence of constipation,incidence of vascular crisis,the survival rate of replantation refers to,doctors and nurses and patients satisfaction.Results: 1 Factors affecting vascular crisis 1.1 Pain score: there was no difference between the two groups in immediate postoperative pain numerical score(NRS)score(P > 0.05),and the NRS score of the observation group was lower than that of the control group on 1-3 days after surgery(P<0.05).1.2 Incidence of constipation: the incidence of constipation in the observation group was lower than that in the control group(P<0.05).2 Incidence of vascular crisis: the incidence of vascular crisis in the observation group was lower than that in the control group(P < 0.05).3 Survival rate of replantation fingers: the survival rate of replantation fingers in the observation group was higher than that in the control group(P<0.05).4 Satisfaction: the satisfaction of doctors,nurses and patients in the observation group was higher than that of the control group(P<0.05).Conclusion: The application of care bundles intervention in patients after replantation of severed fingers can relieve their postoperative pain,reduce the incidence of constipation,effectively reduce the incidence of vascular crisis and improve the survival rate of replanted fingers.At the same time,the nursing mode can improve the satisfaction of the three parties,which is worthy of clinical application.
